Honestly, everything you said about you plan is perfect. I don't even know what anyone would disagree with. You have ED now, you're treating it effectively. You're also worried about future size loss, you're treating it effectively. Physically, from all the credible sources I've read your approach will be effective.
You and I have different psychologies, meaning I had reasons to want mine sooner rather than later, but for you, you know exactly what you're doing.

I often read here that the key to not losing size is to maintain nocturnal erections. Currently, taking 5mg tadalafil every evening, I usually wake up with some morning wood (of varying rigidity and duration), and usually nothing when I don't take it. However, It seems that most people who lost size didn't just suffer from ED or a lack of nocturnal erections, but also had RP and/or PD, making me wonder whether ED alone can really be blamed.
Therefore, I have a few questions:
- Are there any men who experienced size losses purely due to ED (without having prostate surgery or PD)?
- In case you lost size, did you manage to recover it (either through a VED or after having an implant) or is the loss permanent?
- In your opinion, in situations such as mine, when should one consider an implant? Psychologically, I'm definitely willing to wait a few more years until I've really exhausted all other options (except for injections), while in the meantime doing VED therapy to keep everything healthy (a recommendation I got in a previous thread). Physically, however, I just fear that waiting a few more years with waning erections (I have no idea how "strong" and how frequently "morning wood" is necessary to avoid atrophy) would inevitably lead to size losses, even with VED therapy.
In erectile dysfunction with cavernous fibrosis, the size of the penis decreases quite quickly.

noot1991 wrote:Interesting. Did you still have morning wood throughout those 13 years, or did you basically only summon erections using pills/injections "on-demand"? It could be that the injections themselves cause scar tissue and therefore reduce penis size (one of the reasons I don't want to go down the injection path).
I would occasionally get morning wood, but as soon as I woke up to realize it down it would go. Two weeks before my surgery I woke up with an erection after not having one for several months, but it didn't last. At the time injections started failing I felt my length was losing ground. That was two years before my implant, and I'm sure during that time I lost more.
Nice to see a reply from a doctor here. Thank you!

Thanks for sharing! Even on daily tadalafil, mine also goes down very quickly. Worries me because I have no idea what "normal morning wood" is supposed to be like really. Never paid much attention to it either so I don't accurately remember what it was like 10 years ago either.
I do remember though that when I got my circumcision as an adult at age 21 (due to phimosis), I was told that I may experience pain at night or in the morning because of the nocturnal erections stretching the stitches that were placed on the wound. Never had that, because I never had an erection in the morning, and probably also not during the night. Was very happy about this back then; now, not so much ...

Thanks for your response. Is there any way to diagnose cavernous fibrosis?

Your post sounds like a poster child for me but years younger. First, what ever your doctor says. But in my opinion PD is just another form of ED. Here's how it works. Let's say you have venous leakage (as did I) loss of blood flow means soft or no erections and a loss of oxygen to your penis. That is one cause of Peyronies disease. Just a matter of time in my opinion before you have a dying penis. As to using VED yes it will give your penis the oxygen needed to slow down at least the effects. But in 60 days of aggressive VED I made my penile tissue so very soft that it screwed up my surgery. Gave me distal crossover. Also at least 90 percent of those on FT never regain much loss from VED. I was a rare exception got back all 1 5/8 inches. Wait as longer as you feel lucky. Then get an IPP.

Thing is, with PD you typically have a plaque, and the reason for the size loss and curvature is clear: the plaque doesn't extend like healthy tissue (=> causing loss of size) while the side without plaque does (=> causing curvature). Without the palpable plaque of PD, how do you even know that your penis is dying? How do you know you are getting "enough" morning wood/oxygen into your penis to keep it alive?
Your size gains using the VED are very impressive though, and kind of give me hope that losses don't have to be permanent losses.
I'm looking into one as well now, because as you said I want keep my penis healthy and in the worst case be ready when an implant would be the next step. Mind sharing which one you used and what protocol you followed?

Nocturnal erections are normal for many males. Waking up with an erection in the morning, or throughout the night, means you have a healthy sexual organ. Unfortunately, many males who cannot achieve an erection is more common than thought or reported. It takes nerves to seek out help when our male parts fail.

Very good question and here is a great answer. Have your uro do or schedule a Doppler test for scarring, plaque and venous leakage. But don't wait like I did until it becomes excessive. If you have any (3 or more) at all the hand writing is on the wall.
I bought cheap ones VED on Amazon under 20 dollars each. I did 5 minutes under pressure then 5 min off three times in the morning and in the evening.

As was pointed out in earlier posts you can cause damage with a VED. I would guess this is the reason some well-known doctors do not recommend it. And men have been known to over do the use of a VED. If you go to the general discussion section of this forum and pinned to the top is a thread of documents worth reading. That thread has 2 articles on VED use pre surgery. And a lot of other information on the causes of size loss before, during and after surgery. That information will help you select a good doctor for this. You want to be careful using the VED device. Even low vacuum applied to quickly can cause damage. Use low vacuum and apply slowly. The article in the documents worth reading for VED use for penis exercise is very good and worth the time to read it. This has a very good protocol to use. I used a VED with a gauge so I could make sure to keep the vacuum low. I tried for just under 5 inHG but not at first. Take your time to work up to that. The very small blood vessels in the tissue can be damage with vacuum applied to quickly. Another good thing about a VED is that it can be used for sex. But again be careful there are limits on how long and how often. When a VED is used blood circulation is stopped. Blood is pulled into the penis and kept there.
Good luck with this and spend some time reading in the documents worth reading thread in the general discussion section.
