1) I don't know what an "extender" is so can't answer the question. If it is some mechanism to lengthen the penis, I would guess that it would not work with a Titan or an AMS CX, as their length is fixed. With an AMS LGX, which has a variable length, you might get some change. I don't have an LGX so someone else might offer experience with that.
2) somewhere on this board I read that a good doc at Cleveland Clinic will do it for about $12,500 with no insurance. Search around for that post. Be sure to get a good doc as experience counts and you don't want to save money and end up with problems.
3) I am on the west coast so I don't know about Virginia. There is a lot of info on docs on this board. Search for it.
4) Biggest con is that there is a small chance of an infection which would require removal of the implant. Experienced docs get this down to a 0.5% probability. That is one in 200 surgeries, so a very small chance but it is there. Second biggest con is that there will be some pain during your recovery period. Different guys here report being healed enough for sex in as little as 2 weeks after surgery and others not healed up enough for sex until 10 weeks after surgery. How fast you heal depends on your body and maybe on the skill of your surgeon. The biggest pro is after healing up you have a dick that gives you an erection on command 100% of the time.
5) I have Medicare part B, part D and a medigap policy. My entire cost out of pocket was $6 copay for the two prescription meds needed. I looked at what Medicare paid and it totaled about $13,500.
6) From what I have read on many posts here guys lose between zero and 1 inch in length and maybe a little in girth. In my opinion though having a 3 inch dick that works 100% of the time is way better than having a long dick that fails you when you try to use it for sex. A woman's g-spot is only about 1 or 2 inches inside her vagina so 3 inches is enough to satisfy both yourself and your woman. Anything longer is only a visual benefit. I don't have any experience with gay sex so someone with experience might comment on how size matters for that.
In my opinion, the bottom line is there are potential problems and will definitely be some pain in the journey but the end result has a high probability of being well worth the journey. If you can have adequate sex without an implant using pills, injections or whatever, you should probably not do an implant. If you can't I think the high probability of having good sex after healing out weigh the small risk of infection and the pain of recovery.

Thank you Dave I appreciate you taking the time to answer my questions! The idea of the extender is to stretch the tissue then allow it to recover. I think over the span of so many months people gain a fair bit of length. I suppose I wondered would the tissue still be capable of that and would the implant be able to handle it... or even compliment that process seeing as how when even partially aroused it is easier to use from my experience. If anyone reading this has an LGX implant and could shed some light on the extender topic I would appreciate any information. Dave I will search for that board and check around. I'll look into this Cleveland Clinic. I will more than likely end up paying everything out of pocket. I would be in shock if I only had to pay a 56 dollar copay that's pretty fantastic. I hope if I manage to have the surgery that I am in that 0 inch loss group but I see your point. Thank you so much! You were really helpful!
