To make a long story short I've been having issues with my AMX LGX Implant. I've posted before about how my penis is corkscrewed and how the tips feel uneven and very superficial. Both my left and right tip are covered by very little "flesh", so to speak, and I can feel the tips poking into my skin quite often, causing discomfort. Sex has proved painful and uncomfortable-- if the tips of my implant push against her vaginal canal, the tips stab into the little flesh that is covering them, which causes a considerable amount of pain.
Anyways, I finally got around to seeing an experienced surgeon about this, Dr. Jeffery Loh-Doyle at USC (He was NOT the surgeon who implanted me). He examined me and was pretty quick to make a judgement. He said that essentially what had happened was that the glans (head) of my penis was simply too small to accommodate two implant tips. As a result, the tips essentially fought for space, and the left tip won. The result of this is that both of my tips are pushing away from each other, creating the corkscrew and the superficial tips I'm experiencing.
Here's a drawing I made to try and demonstrate what I'm talking about. On the left I've drawn how a penis implant is supposed to fit within the head per my understanding, and on the right I've drawn what is happening with me, where there is simply not enough room in the head to accommodate the tips of penis, leading to my left tip pushing downwards and my right tip pushing upwards.
Has anyone else had this problem? The head of your penis being too small to cover or accommodate the implant tips? Is there anything that can be done about this? Could a revision be done, the tips placed correctly and the head of the penis stretched over to cover them? Would switching to an AMX CX be a good move, because it doesn't expand and perhaps the implant tips would fit better? Are there any surgeons who might have experience with this kind of issue?
I'm feeling pretty doomed about this right now. If I had known that my penis head was just fundamentally too small for an implant I never would have gotten one. Sure I can have sex now, but if it comes at the price of discomfort and pain, what's the point?

The team at USC are well respected, I would think they would have told you what your options are. One thought came to mind while reading your post is that you might try using a VED and a ring which might expand your glans enough to make room for the tips during intercourse and provide some cushioning between the tips and your partners sensitive areas.

During surgery the surgeon is supposed to pull a suture connected to each cylinder through the glans, I'd demand answers to whether or not he did this and if so why am I having this problem.

The term for this is glans hypermobility. Most often used for floppy glans but the term is for any problem of cylinder tip placement in the glans. And a lot of this is cosmetic. For example the tips not being exactly the same length. Many doctors will only guarantee the tips to be within .5 cm as that is the smallest amount of difference in RTE's. So tips not exactly the same length is not uncommon and is accepted in most cases. There are a few doctors that will trim a RTE or proximal end of the cylinder for a perfect fit. Otherwise a slight difference is expected and prefect fit is a bonus.
You have additional glans hypermobility. In the tip placement in your drawing I had this. Only a bit worse. The left tip was slightly lower and to the outside causing pain with sex. The right tip was lower and at one time was close to the center and crossing over. What I did was when the bulb softened enough for one handed inflation, I would use my left hand to hold the tips where they should be while inflating. When I could no longer inflate one handed I would pull the glans forward off the cylinder tips and then push them back down at the proper place. And try to hold them against my arm while I added more pumps. This took a long time and was something I did on my own as I found no help with this from anyone I contacted. The doctor that did my repair stated that tip placement in the glans was like standing a broom handle in a bucket. You need to pull your glans forward off the cylinder tips and when you get the close to where they should be I would deflate only partially to keep some pressure holding the tips in the proper place.
I have some engorgement of my glans with stimulation. This gives me enough room for the cylinder tips. Some men on this site take oral ed meds to help with glans engorgement. You might want to consider this as there are much lower cost options for this now/
I had nothing that worked for me pre op. Mine is not perfect but it works. I had other problems that lead to this. The thing is that it works. Even with problems it works. I have and can have sex again with a woman. I am thankful I avoided infection twice. My size is a just under an 1" from my healthy youth. Maybe within a 1/2" if I took oral meds to help out with more glans engorgement. That could have been a lot worse. My cylinder tips are different lengths when inflating. Sometimes are a while they even out. Sometimes they don't. Coming up on 5 years and it still works.
Try to see if you can get the cylinder tips to go up into the glans where you think they should be and maybe try some oral ed meds for glans engorgement. And have as much fun with it as you want. Good luck.

My distal cylinder tips were out of my glans when I was activated. Only one cylinder would inflate as I started pumping That was the left cylinder and as it inflated and the right did not it moved to the left and down. Once fully inflated the right would start to fill. With the left already inflated to max this caused the left tip to move to the left and down. I cannot say about the size of your glans. That is a question for your doctor. And the reason I suggested oral meds to see about glans engorgement. AMS distal tips seem to be narrower than the Coloplast tips. When I started doing this I did it every time I cycled. I wanted to make a new more correct place for the distal tips in my glans and let the old place fill and heal. I was cycling 1 or 2 times a day. I did take a break every once in a while. I do not fully deflate anymore. If I do my distal tips will work down into my shaft below my circumcision scar and cause discomfort. To push the distal tip up(out) into the correct path you need to be just below where the tip angles into the incorrect path. For me this was at the proximal glans/distal circumcision scar. When I first start pumping I can do so one handed. If I remember correctly that would get me inflated to where the cylindertips were just going into the glans but would not stay in the new path. So I would pull forward(out) on my glans with one hand and move the cylinder tips where I wanted them with the other then move the glans back where I wanted them. Then the tricky part of holding them in place and 2 handed pumping. I would place one forearm so that it could hold tension on the glans and also pump. If it slipped the I would reposition and repeat until I was inflated to max. I can't remember how long I did this but it was a while. It is not perfect but has improved a lot and very little pain or discomfort. Just keep at it evey time and you may have even better results if you can pump to max one handed or have a partner to help. I can pump the pump but after a while I need my second hand to hold the pump in place.
Doctors do not like to reoperate if possible. If the issue is cosmetic they will not. I had pain but no visible signs of injury. I was worried that if I waited for the visible injury signs and then wait for the process to get the doctors to do something that I would be at a high risk for infection. So I tried this. No doctors anywhere close to me would do that repair anyway. They would have just removed my implant and left me with nothing.

I did this in my first year or so after my implant and I remembered that the breaks I took were to deflate fully to give the misaligned path time to heal and fill. I would only inflate trying to establish a correct path. I only had twisting of the glans at max inflation and the distal tips did go pretty far up in the glans. By pulling the glans out I was able to get the tips a bit lower and closer to 1/2 way in the glans. Just wanted to add this as something I remembered. Not sure this will help you as it did me. But it is something you may want to try.

My implant works perfectly, however, like you, my tips do not go into the glans. In fact, I can bend my Glans 90 degrees either way and my tips press firmly against the skin of my shaft causing me worry of erosion. Sex isn't painful per se, but after a period of time, say half an hour, I do get sore.
I have seen another surgeon about this, who initially said i was undersized and would need a revision. I had an MRI but he has now changed his mind and says i am only 8mm short and there is very little chance of erosion. Because of my diabetes he is now not recommending a revision. I get a very floppy head, because the glans are not supported but once I 'manipulate ' them to fit into the vagina everything feels pretty normal, for both of us.

Implanted by Mike Fraser in feb 2022 due to a 80 degree dorsal curvature. 18cm Titan - way undersized. 8’ pre Peyronies, 6’ pre implant - currently 6’.

In my case the distal tips of the implant are both quite deep into the glans, but they are uneven, the right one is usually higher.
The doctor made me see and touch it the day after the surgery, without asking him and he told me that those tips would be competing for space, I have a pending conversation with the doctor to give me more details.
I haven't really seen them compete until now, the right one is always higher and the glans tilts a little towards the opposite side.
I had thought about squeezing the right cylinder a bit while pumping, but I haven't tried it. Reading Newbie443's experience, I see that it could work. I will try something
Right after the surgery, almost the only point of discomfort/pain was that right tip, until the implant was deflated (3 days). Now there is no pain or discomfort, only if the glans does not grow, I can feel the tip a little more

Newbie443, thanks for the detailed response, I appreciate it. I think while we may have had somewhat similar issues, we are in a bit of different situations. I don't seem to be able to pull the glans out as you were able to do, especially not when I'm inflated, but maybe I'll just being too timid and not using enough force, I'm not sure.
I'll keep trying to align the left implant tip when I inflate, but I seem to be having trouble establishing any kind of correct path. Perhaps it's just been too long after my implant surgery to try and reestablish a different path for the implant tip now.
Reading about glans hypermobility on FT, I've run across this procedure known as a glanspexy, which is supposedly a surgical option used to correct it. I'm going to have to do a lot more research and reading to see if this is the path that I want to go down for fixing this issue. Supposedly some big docs like Perito perform them, and I've heard Dr. Levine does them in the Chicago area. Given I am in the midwest, I may try to pay him a visit sometime in the near future.
