I am 42 years of age and have had ED all my life, even as a teenager at 16 years of age. Over the years, I have only managed on a handful of occasions to penetrate women for a few minutes. These days, I struggle to penetrate for more than a few seconds (if at all, as it's not normally erect enough to get inside a woman), before the weak erection is lost completely. Even when I masturbate alone, I have a weak erection (and need a constant stopping and starting technique to maintain even a weak erection), but this has got much worse over the years. In addition, I used to get morning erections when waking up, but in recent years I rarely get these and if I do they are weak. The only time I seem to be able to maintain a weak erection is when receiving oral sex over a prolonged period of time from my partner. But this requires constant stimulation from my partner and even then the erection is sometimes lost. If transitioning to penetrative sex from oral sex, the weak erection will be lost almost immediately. If penetrating, I probably have the most luck in missionary or cowgirl to maintain an erection for a few seconds.
Last year I tried cialis prescribed by a doctor, and even at 3x the maximum dose, it does absolutely nothing. I have now been prescribed caverject (alprostadil) injections and have worked my way up to 30 micrograms (will be trying 40 shortly). When experimenting on my own, it seems to work (about 70% of the time) to get a weak erection, but the erection is lost very quickly, after a minute or so. So much for the story of people having to go to hospital to deal with a 4 hour erection! I wonder if this evidence is pointing to a vascular leak? But then I wonder why is possible on occasions to get a constant weak erection from masturbation or prolonged oral sex?
Then there is the issue of my testosterone. This is low at 203. I look flabby and not very muscular. This is despite an active lifestyle of running 5km 3 times a week, cycling, long distance swimming and weight lifting. I am 170cm tall and weight 93kg. In terms of my lifts, I bench press a maximum of 125kg, squat 150kg and deadlift 200kg. On paper, this places me significantly stronger than the average person, but they are not very impressive lifts considering I have been training for 10 years. If I did not lift, I believe I would be significantly weaker than the average person (I was always the weakest kid in my class at school when it came to shot-put / Javelin). On paper, I am stronger than a few other guys I know who train, but they are viably much more muscular with much higher T levels. I just look flabby and fat, despite good form and a clean diet. I guess there is muscle there, but its hidden underneath my fat. I also have problems sleeping and have sweats at well, which is consistent with low testosterone. In light of this evidence, I wonder if testosterone could be the issue? The problem is because I am in the UK and not below 200, my testosterone is seen as "normal" and not low. Therefore testosterone will not be prescribed in the UK. Reading online, many doctors in the USA regard anything under 300 as low and therefore prescribe testosterone. It seems arbitrary that if I go down to 199 I am entitled to this treatment, but at 203 I am not. In terms of supplements, I am currently taking these in the hope of a small boost in T levels:
L-Arginine
L-Carnitine
ASHWAGANDHA
Maca Root with Ginseng
I have been taking these for a couple of weeks, but can't say I notice any massive changes anywhere (although its been a couple of months since my testosterone was last measured)
In light of my low testosterone, I was wondering if I should try and order some testosterone online? If I went this root, I would not start the testosterone without fist embarking on a massive diet to try and loose as much fat as possible. I may also look at obtaining something like Anastrozole to reduce the changes of the testosterone converting to estrogen. I would also continue the weight lifting alongside testosterone injections.
What do people think about my situation? If I am honest, I want to exhaust all options before I consider an implant.

I would try pills+injection combo. Maybe with 50 mg sildenafil. Take the pill first and wait that your dick reacts to the medicine and gets a little bloated. Then it is easier to shoot the injection.

When ED started, I tried numerous herbal pills/drugs stated to enhance male performance and erections. Not one did anything for me. Viagra worked for a few years, then I had to go to injections. Typical male venous leak that just got worse until nothing worked.

Sildenafil does nothing for me. I have done 60mg before and it had zero effect. I did not get any side effects either, but in terms of an erection it did nothing. Maybe I should try 100mg?
If it is a venous leak, why can I maintain a weak erection when masturbating using a stopping / starting technique? Or why can I maintain a weak erection if I receive prolonged oral sex?

Im 27 years old. I’m actually quite terrified from what i read from you about start/stopping technique. I never thought about it previously but yeah i got it EXACTLY the same. I don’t know why this happens to be honest. Every time i stroke my glans with foresking slowly i can get decent erection. Even close to my maximum size. Then i want to do some rapid moves and in matter of seconds it deflates. If i slow down and keep stroking in specific way - it returns slowly but is often not what was at the beginning…. I read somewhere that jerking off penis alone in the way that most stimulates you is better than having it from a partner or by penetration because you know exactly what stimulates you the most. Considering this - as you and I are at the most peak moments of stimulation we are overcoming outflow by comprehension of good stimulation that provides good/max inflow.

Why do you say don't do it? I was only going to do a cycle to see if it made any difference. I did see one study of testosterone therapy helping men with a vascular leak. If it does not help the ED, at least I may get some weight lifting gains out of it. Of course, I would massively cut fat before going anywhere near testosterone.
If I don't try the testosterone, it seems I am at the end of the road. What else is there left? Tablets don't work, injections don't work. I even tried abstaining from masturbation and sex for a couple of months, but I had the same old erection problems when I eventually had sex. I feel pretty down about it all and don't know what options are left other than an implant.

Have you run through any kind of special examinations ? doppler , cavenosogram.

I have not had any tests done. Only been prescribed callias and sildenafil by the doctor and then alprostadil injections. As I say, I am coming up to maximum does for the alprostadil. Then it will be back to the clinic and they said they will refer me to another hospital for tests. Its all going to take a considerable time. Given my symptoms, I suspect it is a venous leak.

Retired. R.P. 2016. Bilateral nerve sparing surgery. Now use .15cc of Bimix twice weekly & anorgasmia, moderately incontinent. Wife no longer interested so go solo with Electro-stim using Erostek ET-312. Now am Type 2 diabetic.
