If I get an IPP I'm leaning towards the AMS CX.

I was leaning Titan, because I preferred the idea of a smaller pump in the scrotum, but it seems like the dog-ears and crinkles caused by the Titan are a bigger discomfort than the bigger AMS pump.

If I get an IPP my preferences at this moments are an AMS CX installed through a penoscrotal surgery (for optimal pump placement)

I like Dr. Clavell’s analogy:

One’s a Lamborghini and the other is a Ferrari.

I allowed him to guide me to the Titan. He noted that i have frequent sex, and tend to go at it pretty hard sometimes. (the things one tells one’s doctor. 8-) ). After we discussed my sex life he just smiled and said, “ok. Titan for you”. …and i’m not that big btw. (5 1/4” +\- 3 1/2”…mostly minus). I couldn’t be happier.

I don’t think there’s really a “wrong” choice.

Thailandbound's post reminded me that when I consulted with Dr. Clavell I mentioned that I felt my scrotum was a bit small and he said "you do not have a small scrotum".

I'll admit I've never done a study of scrotums (scrotii?) and have only seen maybe a dozen or so of them in my life to compare with, but that may also be a calculation that your doctor makes if the Titan pump is smaller and would fit better in a tighter scrotum than the larger AMS pump.

Again, I thought I had a scrotum that fell on the smaller side, yet my AMS pump is not at all noticeable in my sack whether it's warm and relaxed or cold and tight.

If you want more length, AMS 700 LGX should help. It advertises as increasing length up to 25%, and I can confirm it did increase my length, so I believe you'd very likely be over 5" with good and consistent cycling/VED use:

Pre-op dick size: 8.75" x 5.7"

Current: 9-9.5" x 5.5"

Goal: 10+" x 6+"

Table 1 in this study shows the much larger amount of length expansion of the LGX over the Titan. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5654325/ And if you compare the 21cm LGX to the 22cm Titan you can see that girth is very close to the same. The 18cm Titan and LGX is a much different story in possible girth however. I wish there was a size chart for all the available sizes of cylinders. And I would like to see the CX included. The results are clear though that the LGX did expand in length 3-4 cm and the Titan less than 1 cm. And I would say the reason I had gains out past one year. My LGX is doing what I wanted in restoring some of my lost size.

I know a lot of guys focus on size, but personally I would rather focus on comfort and sensation.

I don't think one or the other matters when it comes to sensation (but I do wonder if penoscrotal surgery preserves more sensation than infrapubic).

However I've seen several examples over the years where someone started with one implant and then switched after the first failure and some of those guys said AMS felt more comfortable inside and I've never read the opposite.

For me, this doesn't pass the smell test (not you 2435tjklAS, the AMS claims) . They advertise up to a 25% increase in length? C'mon man! Increase over what baseline? Flaccid? So if you have a 20cm implant installed you can expect to pump up to a 25cm implant? That's nearly 2" longer for us old school rubes. 25% of the distal portion of the implant? So if you're 10cm proximal and 10cm distal you'll have an additional 2.5cm of growth, or another inch on the business end of your penis?

It just don't make no sense.

Notice that they state the patient will have the "opportunity" to maintain or regain length, nothing about increasing length. That statement is a whole lot of nothing.

This study below, completed in 2018 (not 2010 like some AMS refers to in that ad) concluded this: "Implantation of the length expanding AMS 700 LGX cylinders are not sufficient for increasing stretched penile length." And their results after 12 months of the LGX being installed show "On average, there was a statistically significant decrease in stretched penile length (Figure 2b; 12.5–11.2 cm, P = 0.0016). There was no change in depth of prepubic fat and flaccid penile length across visits. An increase in stretched penile length was seen in only 6 (23.1%) patients" (emphasis added)

Most interesting to me is that AMS and Boston Scientific PAID for this study from 2018, yet they refer to studies from 2010 claiming the LGX will result in up to 25% increase in length, and also claim that patients had a median increase of 3cm in length, are doctors implanting 12cm cylinders somewhere? If the average cylinder implanted is 20cm (I don't know that), then a median increase of 3cm would be 15%.

25% increase makes me think of Inspector Gadget and his go go gadget arms.

There are also many studies showing that the rigidity of the LGX is much lower than the Titan or CX. I can only assume due to this "stretchable" portion of the cylinders.

The best hands down is the coloplast titan XL. The AMS LGX will is not designed to expand in length over time. rather it’s supposed to stretch down to a more natural flaccid. which who cares.
The Titan, particularly the XL is girthier, higher axial rigidity, and higher satisfaction rate.

Yeah I wouldn't feel any need to argue about any of this, cause it's an advertisement and I don't work for them lol. My personal opinion is that "expansion up to 25% in length" is probably a number they came up with somehow. Like isn't that a quote, "there's lies, damn lies, and statistics." So 25% expansion is something they can state and won't be sued, but in reality I do think expecting a 1cm, 2cm or maybe up to 3cm increase in length is reasonable. I have seen those numbers used in a variety of studies (for inches that's 1cm =.39", 2cm = .78", 3cm = 1.18"). And it's also been my personal experience that my stretched flaccid and some measurements erect and aroused has definitely increased by about 2cm. Now that's not a 25% expansion, but it is something. I think I can confidently enough say that if you use a VED consistently and effectively before you get an implant, that with the AMS 700 LGX, the urologist will likely be able to implant cylinders that meet your best, natural dick size pre-ED, and a VED as well as other cycling exercises after implant means this device is very capable of adding maybe a half inch or inch of length. I imagine 100% of patients don't report size gains, but I do think it's probably a pretty high amount who do.
