Advice Needed-Implant

Hello,

I am 32 years old, I have ED for last 10+ years.Never have been in fullfilling relationship, because of ED.

I have been to different urologists here in Germany, some recommended sildenfal, tadalafil and levitra. With tadalafil 20 mg i can get about 30% hard, but the penis han axial rigidity. Also the erections are unreliable with this dosage of tadlafil. Recently, I went to Dr Frank Sommer in Hamburg Germany. He diagnosed me with venooculsive ED. Under his care he advised me to do pumping 4 6 months(5d/week 20 min session) with regular 20 mg dose of tadalafil in the evening time (for erections in night). After 4 plus months had no luck or improvement, also my testetrone was about 370, and they put me on TRT for 3 months to check if the situation with erection improves. Ifelt a little better on TRT plus 20mg dosage of tadalafil but you can say it was only 5 to 10 % improvement. Difference was not great, and thn i decided to stop TRT, because i would like to have kids in future.

During the diagnosis, i was injected with Alprostadil by 3 different urologist during diagnosis phase, with 20 mg i had very weak erection about 40% for about a minute, which was not enough. And after 2 minhs they tried again with 40 mg dosage. This time i had much harder erection you can say about 75% hard. It lasted for only 3-5 minutes. I am not a big fan of needles, so i am confused. I have lost almost an inch due to ED and half inch in girth.

I have consulted with Dr. Osmonov also, he also believes i should get an implant. I have an appointment comming with Dr Sommer also, he will recommend me the same. I am little bit worried about the major surgery, and how it will impact me physically and also emotionally. I can not focus on work and my life in general because of this ED issue. As I get some of my information on Franktalk, can you please guys advise me what to consider before taking this step. And can a woman know if i have an implant?

IUn the end i will have an optio to do an implant with Osmonove in kiel Germany or from UKE Hamburg (unknown surgeon)

My advice would be to go to a surgeon who does hundreds of implants a year if possible.

There is a big difference in satisfaction and success rates between high volume and low volume surgeons.

Greetings friend. We are here to help you. And we all, in our own ways, have suffered with ED, so we can relate to what you’re going through. I’d suggest:

1. Invest a lot of time reading this forum. It’s been here for years so there are MANY great posts about the subject. Learn as much as you can here.

2. Remember, when your penis stops getting hard despite your treatments is the day it officially starts shrinking. I still had some good but unreliable function with injections. I could have waited until “the end”, but i didn’t. This has resulted in being able to maintain length and girth. The “sweet spot” of timing is subjective. Up to you and your doctor.

3. As mentioned above, go to a doctor with a LOT of experience with implants. Their are many potential challenges with any given man, and a high-volume specialist will not be surprised with your unique anatomy. He’s seen it all. Don’t take chances on your only penis with someone not highly experienced.

4. Implants work. I’m 10 weeks after surgery now and having frequent sex. But, it is a “different” experience. Healing takes time. The recovery period will involve you stretching and inflating (“cycling”) your new penis. It’s a time when you will be able to have sex, but you will deal with the newness of pumping. Full sensation will take some time to return, but it will.

5. It is an irrevocable decision. When you’ve had enough of other treatments, when you know in your heart it’s not going to improve, then it might be time.

I’m glad i did it.
Active, athletic 63 years old. Sexually, still 33 in my mind and spirit. Pills and injections all worked, until they didn’t. Diagnosed with veinous leakage in 2022. Coloplast Titan. 22 CM. No RTE. Peno-scrotal. Implanted 1/4/23. Dr. Clavell.

I think Osmonov is the best in Germany and one of the best in Europe. Sure some american surgeons might be higher volume and have more recognition but they are too expensive. With Osmonov you can even get it covered by the insurance. I have talked to some guys here who were implanted by him and are satisfied and rarely visit the forum anymore(a good sign in implant forums). I myself am too young now but when I get to 30 I will get the implant by Osmonov
Venous like since I was 20 years old. Pills don't work too well. Thinking of implant in the future

I had ED and hard flaccid for a little longer than you have had ED. I went for the surgery at 30 7 months ago and do not regret it. Choose a high volume doctor and follow his advice. Im free to talk of you would like.

I had hard flaccid (Potentially Post SSRI Sexual Dysfunction) , turned to long flaccid after priaprism incident.

I am also from Germany and was diagnosed by Dr Sommer. He referred me to Dr Osmonov. I haven't lost any length and even a bit more width. In Europe there are few good ones, for example Ralph, Antonini, Osmonov. I am very happy with my result.
1995, ED since birth due to venous leak. Pills worked but then get
peyronies. 24 cm Coloplast Titan with Dr Osmonov, germany on 2019/21/3

A point to consider to all in a similar situation. Most urologists can only offer pills. This is the limit of their training. If pills are not working then you need to find a urologist that has more advanced training. This generally means one that can or will suggest an implant.
I'm not familar with the European healthcare system. But even in the USA you must be persistent & ask the hard questions of your healthcare professionals. If you just meekly accept poor advice. You'll get poor results.
If it is possible, search for the most qualified surgeon within the distance that you're able to travel for care.

How did Sommer's diagnosis progress= can you explain the steps?

Unfortunately, thats true. Last couple of year everyone was giving me pills. Take 5mg Tadalaflil for 6 months, 20mg/10 mg alternate days, take sildenafil..start TRT. All i can say, the system is supportive of big pharma.

After the surgery did you had any issues with the pump, or any one you know had to go revisions surgeries withn 1 year of implantation. ? I am actually scared of early revisions, but one can never know when the device will fail. That's give me anxiety. Better to have working dick thn no dick.
