I still have swelling and am getting pain 7 days after surgery. My balls look like a leather snake skin and are pealing dead dry skin shaft of penis is less dry but still a bit and whole area is sore.
I would rate the pain a 5 or 6 on a scale of 10. Should I contact the doctor for another round of antibiotics or is this pretty normal for the penoscrotal approach.

Although my implant was done infrapubicly, I would say what you’re experiencing is pretty normal. You’ll probably be surprised the changes (for the better) in another week.
Radical prostatectomy in 2002. Used injections for 20 years. Implanted (infrapubic) with AMS 700 on February 10, 2023.

Yes, I think it's normal to have scrotal swelling (but not necessarily pain) post OP whether the method use for the implant is penoscrotal or infrapublic.
Mine was infrapublic and the swelling gradually reduced over about 3 wks which was when I was approved to begin having sex. I never had any pain but my left "nut" was very tender to the touch post OP and this tenderness did not go away for 8 wks.
Your "mileage" will vary.

Just my opinion, but I don't think what you re describing is at all abnormal. My install was also penoscrotal and don't remember pain to the level you describe, but we are all different and so are our recoveries. At seven days my sac was still swollen and various shades of black and blue and scaly and peeling. In my experience what you are experiencing seems pretty normal.

Mine were swollen for 3 weeks. Ice and pain killers help. Some docs give meds for swelling as well.
34 Years Old...Peyronies for 4 years. 20 Degree left and upwards curvature, major dents and narrowing, ED.

just curious, but how much were you? I seen everything after the surgery?
Personally, I had my entire package virtually encased in an ice bag for the first five days. Consequently, I had very little swelling, and very little pain. I’m sure a lot of it had to do with Dr. technique, but I cannot help but feel that the icing really helped everything. I was amazed.
But from what I have read elsewhere, what you were experiencing seems quite normal. That pain you mention probably needs to be attended to with prescription level pain medicine.
I’m not an expert, but I think there comes a point in time when I seen becomes counterproductive, and with you at seven days you may have reached that point. May be a little online research in this regard.
But I think you’re going to be OK.

Very little icing I kept getting the ice bags leaking on me so I got very scared of infection as Doctors told me to avoid getting the wound wet at all cost. As a result I didn't ice that much because my pain was well controlled.

I'm pretty sure that if you had quite a bit of bruising. Its likely that you'll get some dead skin. So my advice is flaking skin can be a normal part of the recovery from swelling & bruising. If it becomes bothersome. Some gentle skin moisturizer might help. I don't think cracking or bleeding would be normal.
