I can share my experience with insurance. I never tried any oral medicines or injections in the USA. All these treatments for me were done in India. When I came to the US, Dr. Eid examined me and post my penile doppler concluded that I am a candidate for an implant.
My insurance did approve the surgery and I have had no issues till now (been about 7 weeks since surgery)
In short, as far as my insurance was concerned (BCBS PPO California) they did not need any proof of medicines or injections being tried.
42 yr old from India, Ed since many years.
Coloplast Titan 22 cms one touch pump no rte; implanted by Dr. Billy Cordon in Miami on 02-March-2023. Found Dr. Cordon on reco from Dr. Eid after he fell sick.

If you at Perito's site, he offers various additional services, like limo service for p/u at the airport, to & from the hotel that you can book thru his office (or by yourself, personal visits by staff and/or Perito at the hotel postup and a direct phone line (supposedly) to Perito to use if needed.

Of course, this all costs more.

Are you sure you want to go infrapubic?
Born 6/15/74. I have substantial venous leak with fairly severe hour-glassing, but no hard plaques. My urologist is sexual health expert Dr. Laurence Levine who performed a Doppler Ultrasound and diagnosed me with VL in 2020. I also have mild BPH

Why not? I did with a much faster recovery time than pensoscrotal (PS) and no problems whatsoever.

The majority here (including you) seem to have gone w/PS . So, it doesn't surprise me that PS seems to be the most often recommended but, in the long run, the end result is just the same.

So, there's no reason IMO to question the choice. 8-)

I haven't had the opportunity to do it yet, but my choice now would be penoscrotal with AMS CX

I would do penoscrotal just because I have bad luck in life and I would want to avoid any chance of nerve damage no matter how small.

I also understand the doctors can be more exact in pump placement

If that's your choice, that's fine but it doesn't mean that the PS method is "better" than the IP method. It's just what you "think" will be better for you.

If you're saying that reservoir placement by P S is "more exact" than by IP, then this statement is blatently false.

I consulted with both a PS and IP doc before I chose IP and the PS doc told me that he would have to place the reservoir "blind" while the IP doc said that he had direct access to the reservoir site via the IP incision.

This was of importance to me because I had a double hernia operation and surgical mesh could interfere w/the placement of the reservoir. So, IP was the better choice for me in this regard.

He clearly said pump placement, and in that regard he's absolutely correct. I know you know the difference between pump and reservoir, sswinsfba.

A double hernia issue could certainly indicate the better option is to do IP, I'm not a doctor, I don't know what the issues would be, but that's why we do research and engage qualified professionals to guide us.

Mine was covered by reg insurance and they play lots of games in the billing both the hospital and the insurance company. So hard tell. Someone paying cash is very different than going through insurance.

Mine was covered by Medicare and the advantage plan I had with United healthcare, thankfully! ,
The billing that I saw from my insurance provider showed that Perito was paid approximately 18- k , my cost out of pocket was about 1- k with hotel room included in that ! WHEW !!! I was very surprised !

And as far as the procedure, mine was IP, and had almost no issues with pump , procedure, the medical staff at Coral Gables was ( awesome ) ..

His bedside manner after the procedure maybe isn’t so cosy ! But he is an extremely busy and qualified surgeon!

I didn’t go to him to be his buddy, I needed to get my dick fixed ! And he did a great job for me !
69 y/0, ed for approx 25 years, pills worked up until 2-years ago, tri-mix lots of different mixtures, none worked worth a crap, Bad V/l , implanted 4/20/22 dr Perito / Miami -he’s pretty damn good at what he does. titan, with genesis pump 21 cm…
