How to cool and clean your balls in the immediate post-op time?

In a few weeks it will be time and I will have my surgery with Dr Hakky. I am nervous, but optimistic. My thoughts are already circling around the immediate post-op time. What I still don't understand from various posts here is what the optimal cooling and cleaning routine after a peniscrotal surgery is. Would I wear underwear all the time and let the cooling work indirectly via Ice Pads through the underpants?

Should I clean the incisionbwith disinfectant every now and then or is it better not to do anything at all.

Also, I don't understand why jock straps should really be better than comfortable trunks?

First, jock strap. My drs nurse explained that no matter the briefs. Seldom is the penis pointed up sufficiently. I tried rolling up wash clothes that I put between my briefs & my penis. Actually worked fairly well but needed frequent adjustments. My final method was briefs with a jock strap over them. Once I pulled on whatever I was wearing for pants. A pair of suspenders were hooked in the back to the pant waistband. In front, they were clipped to all three waistbands of what I was wearing. Yes, the pants, underwear & the jockstrap. It was actually quite comfortable, kept my penis up & no partial erection was to be seen. Keeping your penis vertical is important & its easy to do.
My instructions were no bathing for at least 3 days. I followed that. Clean briefs every day. After 3 days I was told to use heat. This really helped to loosen the scrotum. Lets the pump & balls seek their own equilibrium. That increased my comfort. Great prep for when your pumping lessons start.
I did leave the house almost everyday. Avoided sweating so I washed the genital region with warm water. At my nurses visits she commented that my incision looked great. Shes been doing this post implant counseling for 12 years so I trust her observations.
As always, my instructions & my results. If your dr varies from this by very much. Follow his instructions.
Don't get overly carried away. My instructions were to avoid strong soaps or antibacterial products. They might cause a rash or micro abrasions. Just clean & dry.
Lastly, be sure to buy at least 1 tube of Aspercreme. It really helped on shaft & scrotal discomfort. The nurse did advise to not put on the incision but I'm sure that at least a little bit spread to it by accident.

I would follow your surgeons directions for cleaning.

I did use gel cold pack to reduce the swelling of my scrotum, but I just applied it to the outside of my athletic shorts. I would turn the ice pack vertical and cover my scrotum and some of my shaft. The ice packs helped a lot.

Never used anything after my implant because at the time I didn't know better. No instructions from my surgeon other than "go use it" and I hadn't found FT yet. Mine turned out just fine. So my guess is no matter what you do everything will be ok. I wore boxer briefs and pointed my shaft down my left leg. Tight pants were out, sweatpants became my norm for months. Cleaning was in the shower, no direct spray on my shaft (ouch).
My guess for cooling is a bag of frozen peas. They will shape to you. Buy 2 or 3, put them in freezer zip baggies, rotate as needed. Rice works well too but don't hold the cold as long.
