My doctor never mentioned keeping it pointed up, and I didn't try until I found Franktalk 19 days later.
I was able to tolerate it at night, but during the day it was just impossible to do anything with it up.
My erect angle is now around 4 or 4:30, but I do have enough flexibility at the base to enjoy any sexual position.
You should definitely consult your doctor, and hopefully he can advise how to make the most of your situation.

For the angle, I think it will take the final shape after cycling to the max for a couple of months. It is very important to make sure that you fully inflate, when it is painless. This will come gradually.

Had my implant on May 2 and my dick points downward. The doctor didn't require me to keep it in an upward position like some do. I have been doing range of motion with it to move it across a range of positions to make sure it remains flexible. I don't think you'll have any issues as long as you make sure to keep moving it in all directions periodically to keep it mobile.
57 years old Bi male. ED for decades. Take blood thinners. Tried pills, shots and pumps. Had Titan implant on 5/2/2023.
