The problem is the surgeon did not pull the cylinders far enough into the gland. The tips should be about quarter inch from the tip of your penis giving like just enough meat for cushioning. Need to have a good surgeon pull them tips well into the gland, then add some rte's at the base to compensate for the loss at the base.
That is why it is so important to go to a specialists that has done many of these types of surgeries. Sorry to hear of such a mistake made by a surgeon who was not competent. Good luck on your revision!

Actually this does not sound like a problem with AMS, but a problem with the installation (the surgeon). What you have described is a spot-on definition or "floppy head syndrome". It is caused by the tips of the implant not being inserted fully into the distal end of the tunica albuginea, thus not fully supporting your glans. The tips should be at least halfway (from the coronal ridge to the tip of your meatus) into your glans.

This is hard. How could you overcome these issues over the past 3 years? Are the 5cm and 6cm in your signature RTEs? Many posts of members here complaining about having 2cm+ RTEs. Only one of 2 members have 5cm RTE, but I didn't see anyone with 6cm RTE.
I am bringing this up just to question the competence of your doctor, putting into consideration your revision only 2 months after the implant (cylinder fell out, which is not a common issue that can happen with any surgeon, even a medium-volume). You definitely need an opinion from an experienced implant surgeon.

If you have poor blood flow most likely a chick would notice, with good blood flow most likely she wouldn’t know in my opinion. If they feel your balls then yes, they would notice the pump.

Age 55, Ohio - Implant AMS 700 LGX, 21+ 3 following RAP for prostate cancer (Gleason 6). Implanted June 2021 and loving it!

I agree with this one. Can you feel the tips of the implant when hard, it should feel like 3/4 in the glands if you have good blood flow. If the glands are not swelled it should almost feel like its right there.
If it feels sized right then there is a lack of pressure for some reason.
I don't have an implant but i am considering one. Does this mean that a casual partner who would give you oral sex would realize you have an implant?
Impossible to say if you’re undersized. With mine, I don’t think a woman would know with mine. Only challenge I would have is gettting a few pumps in. Which I think I could do.
