Just speculating: You have 3 cm RTE which is quite a lot. Reading here usually the guys have 1-1.5 cm RTE on average. Do you know how long your proximal part in your body is? If it’s short, maybe the major part of your cylinders is outside/distal. Your pics do indicate this since it seems you have a relatively long Penis while having „relatively“ short cylinder with 18 cm.
Maybe it would be beneficial for you to get a longer cylinders and less RTE to improve erection angle.
We have the same total length of 21 (I have 20 cm cylinders plus 1 cm RTE). My erection angle is at about 2:30.

I have eaten 20 mg tadalafil a day too . Even inflating with both hands but the glans fails to pop out. THe only thing that has improved is number of pumps from 3 to 9.

I feel your penis is naturally downwards facing - which isn't an issue as long as you are hard enough and able to have sex with it. Before implant you didn't have any erections so I'm not sure if you ever had a straight or upward pointing erection? Even my erection is 3 PM or 3.30 PM naturally, and I am able to have sex. Don't look at porn and compare yourself, everyone is different.

I'm not comparing natural vs implant, you need to understand the context. If your penis anatomy doesn't allow for an upward erection, an implant won't just magically give you one perfectly pointing at 1pm. Anyway, that's just my opinion and I'm not doctor. I was trying to tell you to not just go for a second surgery just because of this and rather get a second opinion, but you have made up your mind. Good luck.

Sometimes a rear tip extender (RTE) or two is necessary for the tubes that connect the pump to the implant to be able to come out of the same incision through which the implant was inserted. Necessary if the incision could be placed more proximally for some reason.
Another reason to pick a surgeon who has done a lot of implants and to make yourself a participating part of your medical care and decision-making team and communicate clearly and firmly your desires. (My opinion.)
