Doppler ultrasound confirmed VL - next steps?

Hi All

So finally had a doppler ultrasound done on the NHS.

Was injected with 20mcg caverject (alprostadil). Managed to get about 80-90% hard for around 5mins or so then back down to around 60-70% for the procedure.

Arterial inflow very good, infact twice the normal amount they usually see at around 83cm/s; however venous outflow at 18cm/s and on the graph was told it should be 0cm/s to maintain the erection.

Anyway now waiting to see consultant towards end of December to discuss results and go through options etc.

Anyone else in similar situation what happened next? Any minor surgeries that can be done? Vein embolism or is implant really only option?

Also had hormone profile done and everything in normal range except prolactin which is slightly above the upper limit at 332mIU/L

I had very similar results to the Doppler exam. Based on info gathered from this site it seems very little success from any venous surgery or embolism. I’m using Trimix and have been since 2019. It is very effective for me. Best of luck on this journey, sorry you’re having to deal with the VL.

Thanks! Unfortunately we cant get trimix in UK, ive read it seems to work much better than alprostadil alone

Try combining the Edex or Caverject with a novelty/sex shop cock ring. The right ring(s), for you, may (possibly temporarily or long term) compensate for a mild venous leak, combined with your alprostadil injections, to give you a satisfactory erection. Expect to have to experiment with some rings and expect rejects until you find what works for you.

R.R.P 2011 Mayo Jacksonville, Dr. M. Wehle. Not nerve sparing. C in margins. Radiation 2023, V.E.D, Viagra and PGE-1 (80mcg/ml) injections @ 8 - 14 units. Originally Edex20, then compounded PGE due to cost. Inject. 12 yrs. It works. Treasure coast of FL.

Thanks mate that's a good idea :-) once I've met with urologist and they give me prescription I'll give that a try and see how it works out, will try all options before going down the surgical route especially at my age but guess it might be only a matter of time

Quite interested to understand why my prolactin is so high too and if i reduced that how it might help

You can get invicorp on NHS which is phentolamine/aviptadil. I find it works better than caverject alone

basically my story, even part about elevated prolactin. maybe because of having elevated prolactin for many years we developed leak or damage to veins which cant trap blood now anymore.

I can have sex with 50mg viagra penetration for 5-15 minutes in 1 or 2 position. on 100mg viagra i managed to be hard for 75 minutes with constant stimulation and walking around my house.

If you would take 100mg of V today, how it would affect you ? Whats ur current experience with pills ?
28 years old. some form of ED always with me entire life.

19th May 22' - doppler, 10mcg PGE1, good inflow, terrible outflow (EDV) of 15cm/s.
Currently about to visit Paris to dr Allaire seeking for further help.

What was your prolactin levels out of interest?

Pills don't work for me much, they make my flaccid much bigger but doesn't get me fully erect

One thing which does work quite well is PT141 but only when I'm asleep which is strange!

Do not perform venous operations under any circumstances. they may help you for a short time, but after that it will be much worse than before the operation.
