Just scheduled an in person appointment today with Hakky for 1/24. Although I am not 100% sure about getting an implant only because I can still get an erection, I am loosing more and more sexual function seeming month by month. After trying pills and injections, I still can't keep an erection after a 10 second orgasm. I can even get hard an hour later but it dies.
However I've been told that since I can still get an erection, I don't need an implant
So here's my current perspective.
If a cardiologist told me I had a 20% blockage in an artery do I wait to have a heart attack to get my heart fixed?
If my knee is in constant pain and cartilage is STILL THERE but almost gone, do I wait until Im bone on bone to get a knee replaced?
If I sneeze 3 times, and develop a sore throat, do I wait until I'm diagnosed with the flu or covid before addressing the problem?
Do I wait until I can't get an erection AT ALL before getting an implant after trying the other methods?
Even after tyoing this I think I just went from 80% to 90% sure I am getting an implant in 2024.

Your hypotheticals are well-considered.
I too had some function at the time of my implant. I could take a massive dose of trimix (WAY more than the normal dose of most guys) and get a solid boner for :20 minutes or so. Then, veinous leakage would occur and that was that.
Sex was rushed and unfulfilling for all if time ran out.
11 months postop. The recovery was a journey. Mind and body had to adjust. Now, i’m loving it. Happy i did it.

Active, athletic 63 years old. Sexually, still 33 in my mind and spirit. Pills and injections all worked, until they didn’t. Diagnosed with veinous leakage in 2022. Coloplast Titan. 22 CM. No RTE. Peno-scrotal. Implanted 1/4/23. Dr. Clavell.

maybe "need" is the operative word. You may not "need" an implant, but an implant would likely make your sex life, and maybe even life in general, much better.
You're here on FT, your lack of ability in the erection department is clearly on your mind. Some men have ED SOLELY because of what's on their mind. Apparently this isn't you, as you've been on pills and injections.
I could take the pills and have a serviceable boner for PIV sex, but the headache, backache, flushed face and miserable general feeling the next day was quickly making it not worth it. I was scared that sex with the love of my life would ever become not worth it... that was my wakeup call. At 58 I wasn't ready to call it a day in terms of intimacy.
I don't know of anybody that has gone down the pills and injection path and somehow recovered and returned to normal erectile function. I believe (I'm not a doctor) that it's a one way street. The question is how far down that street to you want to go before making a hard right turn.

By 2016, after more than three decades of declining erectile function (pills worked, but lost effectiveness) I was at your stage (but eschewed injections) and when I found I could get an implant, I did not hesitate one minute to say "Yes, sign me up." I did take another 14 months to ensure I had a surgeon I trusted, but I never had a moment's hesitation and the only doubts I had were about the permanence of the implant (no going back and being certain there were no less invasive/destructive solutions).

You say your erection fails after a 10 second osgasm. That is natural. Are you watching porn where the guy stays hard after orgasm.That is a film splice or the guy is using trimix. If you want to stay hard after an orgasm, use trimix. You do not need an implant.

I was in a very similar spot as you when I decided. Pills had quit. Trimix was still working but at an ever increasing dosage. Injections were like dealing with ED every time I used a shot not knowing if it would be enough or how long it would last! My mental state dictated it was time as much as my body. About 15 years of pills and shots had had a toll on my marriage…. Sex wise anyway. The frustration was never ending and very depressing!!! Only you can decide when you’re ready for an implant…. But, by the sound of it…. Your there! Good luck. Get the best Dr you can get and never look back! Going thru with it and the recovery afterwards is a lot mental!! Make a decision and stick it out! Maybe I’m the odd man out but, it took about as long for me to adjust mentally as it did physically to having a dependable rock hard dick that want not going to let me down. With 6 months all was better than ever!!
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast 20cm and 1cm RTE. Going strong and loving it!

Just my thoughts on an implant. They are not always about your declining or lack of , sex life. Sometimes they for your mental well being.
