I have battled this issue before and after implant. In my case its a mental thing. I tried fillers thinking a thicker dick would help, it did for 6 months, and maybe still helps. I tried no porn, all sorts of “natural “ vitamins and of course the standard prescription vigra Cialis.
I still have this issue once in a while. The thing that helped the most was getting in better shape, strength and lungs. Then I started masterbating to the female body instead of some fantasy. The last thing that helped, more sex. If you know 4 sure when i go to bed my wife will give it up happily, thats made the biggest difference, a fun loving wife who is dtf. I am highly interested in this subject and would love to hear from others who have had success .

I had the same problem and my doctor said use Cialis for daily use (now a generic and cheap), or Cabergoline, a supplement. Both helped a great deal and since the Cabergoline is so inexpensive I opted for that after a time. It is frustrating to be unable to ejaculate, especially during oral sex when my wife's exhaustion is apparent. Now 2 years out, the problem is much better. I think it also helped to regularly ejaculate (2-4 times for week), one way or another.
