This seems to be something that crops up time to time.
I have mild to moderate ED, but one thing I tend to find that differs online to reality is maintaining erections with no physical stimulation.
Now as far as I was aware other than in your teens possibly maintaining erections without stimulation wasn't normal.
Pretty much every one I have spoken to outside of the Internet has said that when physical stimulation stops their erection goes down, yet I see it talked about online as though that's ED.
I can get a full erection and so long as I'm not in much pain from cpps or under too much stress I can keep it during sex... Although stimulation has to be just right.
Urologist doesn't think I have venous leak although I haven't had a doppler done.
Surely if I had venous leak. I would lose it regardless of stimulation.
If I stop all manual stimulation my erection will be down within 30 seconds regardless.

Sorry, but that sounds like ED and VL to me. However, I am not a medical professional. Just a long-time sufferer of it.
ED worsened over 25 years, likely VL. Went through pills and injections, and results faded over time. Implant AMS 700CX, 21 cm, no RTE, on 6/3/24.

How are you supposed to keep a erection without physical stimulation though? That's the very thing that causes most erections. If it were VL wouldn't it still go down when having sex or being stimulated?
I don't think I nor anyone have asked can keep an erection just standing or sitting with no stimulus, yet online it seems people think it's VL.
Urologist said VL is unlikely or it would go down as quick during sex too, as surely if the mechanism that traps the blood is not working it's not working regardless of stimulus.

Estas sano. Es normal perder la ereccion si dejas de estimular.

When I was 18 or so, actually starting a good deal younger, like most guys I'd get boned out at the slightest sexy thought or image of a attractive female, clothed or not. Heck, if my jeans slid against the head of my dick I'd get boned out. In school there was the fear of having to go up to the chalk board sporting a boner. Later, with the added stresses of a busy life things tapered off a bit, thankfully. There are plenty of times when a spontanious boner can be an issue. Things change as we age. And that's normal. I always had a strong libido but it was nice to have it back off enough so that it didn't otherwise interfere with life. Not everyone changes at the same rate as we age. I'd be nice if we always changed at our desired rate, but not always the case.
I'd say the scenario expressed in the OP may be normal depending on age. Of course at any age, if there's no physical, visual or mental stimulation, a natural boner is going to diminish fairly rapidly.

“Venous leak” isn’t an all or nothing thing. It can be super mild to really severe or anywhere in between.
It is simply the venous system in the penis being unable to trap blood as effectively as it could to maintain an erection.
Have really mild venous leak, you might never notice or just you occasionally have a little trouble with your hardon.
Really severe and nothing works. Not pills, not shots, nothing.
But there is the entire spectrum in between.

Reading this, i can't help but be jealous.
I've never ever had a normal erection function... When i was a teen i did get some night wood, sometimes waking up pretty hard for no reason, it lasted for a few minutes on it's own if i didn't touch it (i did some experiments to see how it works) and i could easily masturbate to "success" if i was quick enough. It was very frustrating tho, since it would go down after a while no matter if i left it alone or masturbated, so if i wanted to "capitalise" on it i had to be quick.
Then, when i started my mid-twenties it all subsided, i was never sure if it's age or the fact that i was with my ex-girlfriend or later with my wife and we were fucking all the time so "i no longer needed it"...
Growing up with a non-functional penis is very frustrating. Always wondering "how would a normal guy be right now?" when you are excited to the edge, about to pop and your body is just not reacting at all... Hell, even having a retrograde ejaculation since forever was very confusing, it took me a while to be "sure" that i've had an orgasm. Guess girls also go through a similar thing, especially with the myth of the "vaginal orgasm".
40 years old, married. ED all my life because of spinal cord injury caused by a tumor in early infant age. Using standard EDEX20 since 2007. Increasingly bad results with EDEX in the last few years, but had very good results for at least 10 years.
Re: Maintaining erection with zero physical stimulation
I have to laugh at bldoink’s post as it reminds me of when I was a young man. If we had known back in the `70s that you need to go to ER for erections lasting over 4 hours, I would have been going to ER all the time. My brother-in-law said we had “perpetual hard-ons”. Those days are gone.
But seriously, I feel so bad for your situation Hunchback. Prior to my RP, which caused my ED, I could get an erection whenever it was needed or desired, but there were times when it would wane a bit. I don’t know if this will help you, but I explored the feelings and sensations associated with getting an erection. I could go into great detail of what I found about myself, but what may help you or others is when I started to lose my erection, I would make myself feel like I was going to have orgasm and ejaculate, which is similar to the same feeling as urinating. I would push real hard and would keep telling myself, this feels so good, I’m going cum and my body would react by making me hard again. If I kept repeating the feeling, I would achieve an orgasm.
Now that I have ED, I have attempted this many times. I feel all of the sensations and it actually feels like I’m getting an erection, but unfortunately it doesn’t move at all. Pills don’t work for me and I have a Dr Apt this month to explore using injections. Until then, I will continue to use a VED doubling up on the restriction rings.
Good luck brother! We are all in this together!

I also can get an erection, but I need stimulation to keep myself erect. If I am not actively stroking or having sex as soon as I get an erection, I do tend to lose it if I don’t stimulate my penis. I can relate to the other guys that posted here. I do have Viagra and Cialis. I noticed I don’t lose it as fast if I take either of those. I’d love to chat with others that are in a similar situation.

.......really ? i could hang a towel on mine and walk around like that in my 30s . and if we were going to have company over, she better not start anything otherwise they would be looking at big ol woody
its going down because you have vl . you have blood flow in but it's not held in without stimulation, if you lose it during sex it's getting worse . Having the girl ride you is the first position most guys start loosing their erection in .
