I got my implant "early" compared with many men here, and skipped straight from pills to implant. Almost two years later, I am pleased I did it.
I could generally get hard, but it would often go down and stay down. Over the years, this was true even as I increased up to the maximum dose of Cialis. I started with Viagra but switched because Cialis lasts longer. As the dose increased, so did the side effects. I would get a really stuffy nose and sometimes a bit of a headache. For my girlfriend at the time, having a cock right near her pussy helped her orgasm, but she didn't care much whether it was soft or hard. Hard while I was still in her was fine, soft after I finished, or soft simply because it went soft — all were great for her. So I did the implant for me. I wanted to be able to get hard without side effects, whenever I wanted, and stay hard as long as I wanted. Now I do.
I skipped from pills to implant. I didn't want the muss and fuss of injections or pumps. I especially didn't want something that had to be refrigerated. I like to travel and I wanted to be able to conveniently have sex whenever and wherever. At first I was thinking of implants as a "last resort", but the more I learned, the more I questioned that. People here who had them seemed happy, even thrilled. Many said, I should have done it sooner. In the old days, the "official recommendation" was apparently that you shouldn't try an implant until you had tried everything else. But more recently, that has changed. They still recommend pills first, but beyond that, an implant is considered a reasonable choice whether or not you have tried the other things. (This is based on the guidelines from the American Urological Association.)
I can't say what's right for you, but for me, going straight from pills to implant was the perfect choice. Knowing what I know now, and having the positive experience that I have, I wish that I'd learned about it sooner and had the opportunity to do it earlier. I'm about 60 and I wish I had done this at 50 or even 45.

There is a questionnaire that's internationally recognized as an indicator of ED severity. I was asked to complete one prior to my consultation with my first urologist. Here's a link to it, just from a random Google search result.

2/22/23 AMS 700 CX 21cm + 1.5cm RTEs. 58 yrs old, wife of 37 yrs. Penoscrotal. 100ml Conceal reservoir. Dr. Clavell. Pills failing and went right to implant, skipped the injections. 12 mos. later: 7 1/2" x 5 3/4"
Re: For all men who have an implant how many had severe ED?
I think that questionnaire is a rather flawed tool as it does not have any options of "never", you cannot answer it truthfully if you cannot get an erection at all.
I pulled a muscle in my groin about 13 years ago, ( which my urologist said must have been incidental, and cannot have been the cause), and never had an erection again, pills, VED, injections and now an implant, and still cannot get one!

Onset of ED at age 46, no erection since, pills, VED , Injections, no luck, not even once.
Implanted 19.04.2023 in UK, with Rigicon Infla 10 AX, not functioning so far, still hopeful.

I'd say the questionaire is flawed for other reasons.
For example I would score 5 in a lot of those because I only actually have sex when I'm hard.
It would be more appropriate if my partner spring it on me more suddenly rather than me being in the mood to start with.

You have an implant and can't get one?

I thought you meant early in terms of age, then I realised you meant early in terms of Ed severity.
I'm 38 and seriously considering it despite being able to get a decent erection.
The stress is outstanding.

Well....uh.....then I don't think you need a questionnaire.

Re: For all men who have an implant how many had severe ED?
I think there are 2 variables in this equation: 1) degree of ED and 2) sexual demands of your relationship/lifestyle.
I have moderate ED that responds to meds but out of preference I prefer Trimix injections. This has fewer side effects for me and less chance of going limp half way through the deed. My wife is not very sexually demanding so I do t feel compelled to go for an implant. However, if I ever find myself divorced or widowed my first appointment is with an implant surgeon.
So as long as I have a mostly working erection I’m good with it. But if I ever find myself single i would be immediately implanted and ravaging every available vagina in a 10 mile radius.
