I agree with my brothers who have replied already. It's not uncommon for insurance companies to deny coverage initially because most people walk away and leave it at that. Appeal with documentation. As another member wrote, the doctor must attribute your situation to a physical issue. Make sure your appeal is not verbal but documented. Insist that the insurance company reduce to writing under what clause they are denying your request for coverage. Also, make sure you know to whom your appeal should be directed. Don't accept rejections from "the Coverage Team" or "the Committee" or "the Benefits Department." Get a name.

Also, if the implant itself is "broken" as you wrote, there should be no cost for replacing the implant itself, only the surgery. (By the by, your surgery can be much cheaper depending on where you have the surgery. For example surgery centers are less expensive than hospitals.)

Thanks for the replies. They told me for paying myself it would be around $25K if done at their place. If I did it at the hospital I would only pay the doctor's fee of $4k and the hospital would bill me. But I don't have the money to pay the hospital even after. So I'm going to call back tomorrow and find out what I need to appeal it and try that process. Thanks for the feedback and I will keep you posted!
