Hi all, I had a radical prostatectomy a year or so ago and have since had a titan implant (May 25th 2016). One of the implant tubes isn't attached inside the glans correctly and they told me today I could have revision surgery for this. I've noticed also that blood doesn't get to the end and the glans stays shrivelled up and cold during sex. My wife has told me she doesn't feel it enough so it's left me totally devastated and don't know what to do. They told me that even if I had revision surgery the blood still won't get to the glans as it's just an anatomical problem.
I heard via Dr Eid though that if the implant isn't in correctly this can also cause the blood from not engorging. So this is totally going against what my dr told me today. Has anyone got any experience in this or had this happen to them?
I've been given 100mg viagra but it doesn't seem to work so really at a loss after all the pain of the surgery to have a dick that doesn't work well enough. Very down and disheartened :-/

Somebody is spoofing you. The blood supply for the glans is entirely independent of the supply for the corpora, which is usually destroyed by implant surgery. Lack of blood supply to the glans suggests a screwup in the surgery, and you ought to hold the surgeon accountable for it. He ought to at least admit what he did wrong.

I agree with headout2000, somebody is telling you some tall tales. There are 3 erectile chambers in the penis. 2 of them are called the "Corpora Cavernosa". They are the chambers that are on the left and right sides of you penis and they are what allow you to have an erection. When you get an implant the erectile tissue is cleaned out of these 2 chambers and replaced with the implant chambers.
The 3rd erectile chamber is called the "Corpus Spongisum" and it is on the bottom side of your penis and runs the length of it. The "Corpus Spongisum" is what provides blood for Glands Engorgement.
I have attached a picture which might make things a bit easier to understand.

I guess this means that, if we need to, we can still take Cialis with an implant. It would help the glans perhaps?
62 years old. ED for years. High BP and meds have done me in. AMS 700 CX /3.0 cm RTE Implanted by Andrew Kramer on 10/12/16. Involved revision to relocate tubes and pump performed 12/29/16 by Dr. Knoll of Nashville, TN.

You had me wondering there for a minute ... was it really spelled Spongisum!!?? Was I always spelling it wrong?? But there's the picture right there with Spongisum....
But no .... it really is Spongiosum.

Getting back to the original problem of no glans engorge. Can you feel the Spongiosum all the way back under and between legs? I wonder what the surgeon might have done wrong if really no blood flow. If just very diminished bloodflow that just might be your poor cardio/circulation?
I wonder if they make a VED cylinder and one of those ring shaped cushion rings that fit on the end of the tube that could be used JUST on the glans... That would be pretty cool. (And and a not too tight ring to slip on below the glans like Island Rings) ... Maybe you can invent something?

With me glands engorgement is kind of a hit or miss thing. A couple of times I have taken some Viagra and it seemed to help with the engorging my glands.

In spite of the normal functioning of the device all of them complained of a degree of dissatisfaction during SI due to a lack of engorgement in the glans penis. The patients were advised to have a dose of 100 mg VO sildenafil about 45 minutes before activating the PP and starting SI. All of them followed this scheme at least three times. They ticked questions 7, 8, 13 and 14 of the International Index of Erectile Function (IIEF) questionnaire with/without having had sildenafil.
RESULTS:
Twelve out of thirteen patients (85.7%) indicated a more pleasant SI on sildenafil. It was related to an increase in penile glans engorgement and sensitivity. The scores obtained in the IIEF questions showed a significant increase on taking sildenafil. The partners also talked of a more comfortable SI that they related to a less painful penetration. Sildenafil-related morbidity was not found.
CONCLUSIONS:
Sildenafil could be used to improve functional results of the penile prosthesis in those patients with CGS and seems to increase satisfaction during SI.

Thanks for all the replies. The physician was explaining yesterday saying the corpus spongisum stopped before the glans in my case so the blood flow stopped after having the implant. She said each person varies but in my case it stopped just before the glans. However I was also told prior to this that if I had a revision the blood might come back to it again. Very confusing. Maybe I should get a second opinion from someone but don't know where to turn as I'm based in London and haven't got enough money to go privately. My gut feeling is to get the revision op and see if it helps?

Oh another important thing I forgot to add was that when I had my consultation they didn't have my notes. How could they know enough to tell me this without notes?

I got in touch with the clinical nurse specialist and she's upset they say none without my. It's so am getting another appointment in the next week or so. Hopefully the doctor who did the unpacking can shine some light on things. Just wanted to ask others that have had the op...I can't feel a corpus spongiosum at all as it feels it's been taken out and just can feel the implant. Doesn't feel anything like it used to before the op. Is this the same for everyone else?? I know they hollow out the other parts but the middle bit underneath is also gone.
