AMS LGX

The AMS LGX has the capacity to expand 18-20% in length over time. Does it expand from the original inflated length after surgery or just from the flacid length after surgery?
Implanted by Dr. Kramer January 2017 with AMS LGX 18 cm with 4 RTE due to peyroines.

The length extension is purely a result of the design of the implant. AMS limits the radial growth with embedded fibers. When the girth expansion reaches the limit of those fibers, all of the hydraulic force is the moved to forcing the tips forward until they reach their limit. That limit is approximately 20% longer than the partially inflated cylinder. My new implant is partially inflated, so the cylinders are full and straight to avoid space for scar tissue to build. However they are not at max girth. When I begin to pump I should see expanding girth then length as cycling and pain permit, That is why it takes a lot of guys 6 months or so to get to maximum.

Not to get to detailed, but does that mean the surgeon calculates the final, maximum (+18-20%) length of the implant and correlates that to the length of the penis in a stretch test? Not quite sure how the LGX is sized if it expands up to 20%.
60 yrs old. Gradually worsening ED for 10 years. Pills and trimix not working well anymore. Will need an implant in the near future.

I can give you a well educated opinion on that, but just my opinion based upon my experience. Keep in mind that the girth will mostly expand first (Pascal's law....a fluid under pressure acts equally in all directions and at 90 degrees to the contact surface) Force = Pressure x area. The area available in girth is WAY larger the the area of the tips the increase length. When your surgeon measures he is stretching your flaccid penis with the measuring rod. I my case and most of his that I have seen, Dr. Kramer fills that space as fully as possible. After healing, your implant can stretch the tissues until it reaches its limit or you reach yours through pain or discomfort. That is why some lucky fellows are able to gain length over time by cycling. Just like a VED, regular applied pressure stretches the tissue over time.

The surgeon should NEVER base the sizo of the implant based on any external measurements of the penis. ONLY measurements on the INTERNAL space of the Tunica Albuginea shoukd be used to size the ijplant.

Having said that, I also am uncleat about choosing the size of the LGX once the size of the space inside the cavity is dretermined.

May I suggest you view one of the videos on the internet of measuring for penile implant? They use a fancy measuring stickk to measure the penis from the incision to the tip (distal measure) and then from the incision to the crus in the pelvic bone (priximal measure). Then add the two measurements together.
