I am a relatively new user, but was having limited success with large doses (50 units). If you want to try something, my urologist did a second test with me and recommended these techniques :

1. Inject at "10:30-11:00" and "1:30-2:00" if you have been using 9 and 3.

2. Inject about 1 inch behind the glans instead of close to the base of your penis.

3. Feel for the piercing of the tunica - the "second level" of resistance as you inject to make sure you are in the corpus. If you do this, I personally don't think you have to draw back on the syringe to prove that you're in the corpus.

4. Gently massage the injection site for about 2 minutes after injecting to distribute the medication throughout the corpus.

Using these techniques, I was able to reduce my dosage to 8-9 units with 100% (maybe 110%) erections for 2-4 hours.

I am on an "unusual" very mild Bimix - Phentolamine .5mcg / PGE1 20mcg per ml.

I agree with injecting 1 inch or less behind the Glans, I pull back the skin exposing the "thinner" layer between the Glans and the "band" and inject.
You know you've hit the spot when the plunger push is smooth and easy, push slowly. Apply alcohol pad to injection site for at least 2 minutes and massage. Injecting in this location rather than near the base results in less "residual" pain for me and seems to distribute the medication better and I use less units.
Use Super Quadmix due to severe Venous leak.Have a GREAT DAY!

Thanks I am going try higher. I have been in the middle 1/3 but closer to the base. I have no problem with the pain of injecting but the pain a half hour after injecting.

When you say you have a 2 hour erection, does that mean hard enough for intercourse for 2 hours? I start going soft a little pass the 30 minute mark which makes me worried, which means a bad sex experience.

Yes, when I said 2 hours it's a 100% hard erection - sometimes a little uncomfortable, it's so hard (the pain that a lot of men experience). If I have an orgasm, I'll soften slightly, but still hard enough for intercourse. But as soon as I'm stimulated again, it gets very firm again. In fact, I have to be very careful of not injecting more than 11 units, as then my erection will maintain for more than 4 hours. I haven't gone to the ER in that case - I've read here and elsewhere that the "magic" 4-hour mark is conservative, allowing you for time to get to the ER and for them to reduce the erection. After 4 hours, I take 120mg Sudafed, use cold compresses to be a little more comfortable, and try a little light exercise - knee bends, a few minutes on the treadmill, etc. It usually takes another 15-30 minutes to start subsiding.

It's a double-edged sword (no pun intended). I am much harder than pre-surgery and without injections - my wife is quite happy, and frankly so am I. With a 2-hour erection, we can relax, spend time fooling around, and then go for the main event - sometimes twice, once orally and once vaginally.

I think that once you find the right technique and the right medication formulation, you can be happy with the results and hopefully with minimal discomfort. It seems that the consensus here on the forum is that who your Urologist is makes a big difference - they may be familiar with dosages and formulations, or may just have the barest minimum knowledge of this. I realize that I'm in a big city (L.A.) with lots of doctors and there are places in other states where there may be only 1 or 2 Urologists near you. But others have suggested contacting one of the larger reputable compounding pharmacies and see if they can tell you who they have worked with. You should NOT have to go to expensive men's clinics to get the help and formulation you need. If your urologist doesn't seem to be knowledgeable, ask him or her if you can consult with the pharmacy and allow the pharmacist to help with formulation recommendations which then the doctor can order. I myself am on an unusual Bimix (Phentolamine and PGE1), and at my next followup next month, I am going to ask for a Trimix with less PGE1, which based on what I've read is the primary ingredient that causes pain. I'm hoping that adding Papaverine and reducing the PGE1 I will get similar results with a little less discomfort.

Keep experimenting, man - it seems that there is quite a variation among users. Some guys need to inject a larger dosage, some need stronger formulations. If your erections are of short(er) duration, my reading is that it generally indicates you want more Phentolamine in ratio to the PGE1/Papaverine - that's the ingredient that has the most effect on duration. Educate yourself - read about formulations, the effects of each component of the mix, and switch urologists if you're not satisfied with your current one (if you are able to). And TELL your current Urologist why you want to switch. At least here in LA, doctors want to improve their patient service, and if THEY need to educate themselves better to do that, they want to. It should not be that difficult to find the formulation that works, but when there are forums like this, with dozens of posts asking the same questions, there's apparently a lack of "consistent" knowledge by the medical community.

I am dealing with the pharmacist along with the doctors. As I started they have changed my formula 4 times. and I am approve for another one. I just need to call them and tell them to send it for $25.00. I use a Papaverine/30 Phentolamine/4 and PGE1/40 or 30/4/40. I am just glad I have no problem with needles or i would have stopped this months ago. I think i am going to get a 30/6/100 and if that doesn't work I am going bionic

Did you have radical prostatectomy?

Have you tried using a constriction ring with injecting? Could make a big difference. Put it on right after injecting. Make sure you can still remove it when erect (adjustable type).

Yep - Robotic RP in Feb 2016. My uro wanted me to try oral meds (Viagra) for awhile, so I got my Bimix 11 months post-surgery. BTW, contrary what you might think, oral meds counteract injectables, so don't use them on the same day. After a month of injecting, the Viagra seemed to have an effect on nocturnal erections, and manual stimulation seems to be getting more of a reaction (ussble erections a few times). Had I known what I know now, I would have asked for it much sooner after surgery.

Why would they?
I use daily Cialis generic 10 mg for enlarged prostate symptoms (it is indicated for this as well as ED) and have zero issues with injecting. In fact, I would think it *could* even enhance effects of injecting by increasing glans blood flow, which is not enhanced by injections. I know that my nocturnal erections are enhanced using daily Cialis.
BTW, I read study of men who had RP surgery and sub par results with injecting alone. They experienced enhancement with the addition of Viagra with injections.

I had my surgery in February of 2016 by Patel in Celebration. I am just like you I started the injection 6 months after and the only thing I have gotten with use of Cialis is nocturnal erections. I have been doing everything that you and everyone on here as said to do but the location. I have been injecting closer to the base. You are using a bi mix right? I don't know why they didn't start me with a bi mix. I have read the with those of us that have had radical prostatectomy the PGE1 causes more pain that men who still have their prostrate.

What does you uro tell you now of your chance to regain natural erections. I dont think that the success rate is as good as they like to tell you. I am glad had it I am in the zero club and I have even better consistency than before the surgery because of my large prostrate. I am giving it till the 2 year mark and then I am looking at going bionic.

You are taking Cialis generic daily and injecting? Tell have told me that was a big NO. I take Cialis generic 5 grams but not on days I inject. I am telling you as I didn't when i started this post this whole thing has been a nightmare for me.
