Trimix crystallized after freezing

My trimix crystallized after freezing. Brought to room temp and tried shaking the hell out of it and still crystals. And the potency went down to zero.
Call Uro who called compounding pharmacy. They wanted all the info numbers on the bottle.
Bottom line is they said they had several vials from that batch that did crystallize. They sent me a new bottle.
I was thinking that freezing it was the problem but when I got the new bottle the label right on the bottle said "freeze after use".
We'll see what happens. Now I'm worried about freezing it.
