Why are your injecting only Aprostadil and not a standard Trimix formula?

I don't know, that was what my uro started me on. Is the Trimix less "traumatic" ?
And is the effect/results better ? I only got a 30% erection from the aprostadil.

The pharmacist at compounding lab and my URO told me that the aprostadil is the cause of the pain. It is common and affects 30% of patients. Lab said to try bimix which has no aprostadil.

Speaking from my own experience: Edu-ma-cate yourself about injections: types, formulas, etc. Use that info (take notes with your if needed) with your urologist. Understand not only WHAT the doc is doing and the choices being made, but also WHY. Since you've now had a booboo with the first formula, your opening line to the uro should be "This is not acceptable" Then go from there. If the uro in not cooperative and forthcoming, get a new one. This is not medical science; this is taking control of your health and your most cherished body part. IJS

All the best.

In Canada, if you start questioning your doctor, he or she might become offended and kick your ass to the curb. Then you could be waiting for a long time to get another appointment. Asking questions id different than questioning them. You must have a lot of doctors to choose from.. :) I am not a doctor so I leave the decision making to the professional I chose to treat me. I think that is still taking part in my health care. Also, if my doctor says not to cycle for 6 weeks, I don't try to do it in the first three days because there is a 1% chance I will be hung like a horse when I'm 90 years old.

cassie8 wrote:As mentioned in my first post on Friday I experienced pain. I penetrated the urethra and lost some of the .25ml Aprostadil I was supposed to inject. On a scale of 1 - 10, the pain level I then experienced was about 8.5
Today I again used the Autoject Ei and injected successfully this time on the left side. Got an erection within 10 minutes together with an unbearable pain, a 10+ on the above scale. It's now 3 hours later and my flaccid dick is too sensitive to touch. I can only handle the pain if I lay flat on my back. The moment I got upright its a 9+ after 3 hours - that is after I swallowed a Trameset (pain) as well as a Advill CS (Sudafet)

Please tell me that some other medication than Aprostadil wil work painlessly ? I'm a man and I know what pain is (had 4 back opps) I will accept a little bit of uncomfort/pain but not this!

Something else, the moment when I laid flat on my back I noticed the erection also started to diminish, that's only after about 15 to 20 minutes

I'm so disappointed - is it over for me? I'm cured from cancer, but is this the prize I've to pay?
Sorry I sound negative, but this depressing.

to me this is very indicative of the pain that roughly 33% of men experience with alprostadal. i too had the EXACT circumstances as you described. Doc put me on Bimix with no alprostadil. that was 2 yrs ago. i am now using it in trimix but at a low level 7.5mcg. it seems my sensitivity to it has reduced. diminishing erections while lying down could be venous leakage. again, the same issue i face.

also autoject is ONLY alprotadil, and could be a high quanity. do to bimix or trimix with very low alprs, about 2.5 mcg..

You're probably thinking of Edex, a Brand name alprostadil that comes in proprietary dual chambered syringes. The Autoject is a syringe automatic injector that contains whatever you load into the syringe you put in it.
R.R.P 2011 Mayo Jacksonville, Dr. M. Wehle. Not nerve sparing. C in margins. Radiation 2023, V.E.D, Viagra and PGE-1 (80mcg/ml) injections @ 8 - 14 units. Originally Edex20, then compounded PGE due to cost. Inject. 12 yrs. It works. Treasure coast of FL.

Just my 2 cents, but I really don't trust doctors, or most anyone trying to sell me something.
I also wonder why a doctor would shoot straight alprostadal and not tri-mix, unless you are in a country that doesn't allow it.
If you're in the US, I'd go with Antelope's suggestion and run, do not walk, to another doctor.
I hope the doc doesn't say "oh you're getting pain, why don't we give you an implant".
I'm not downing implants by the way.

cassie,

I see from your profile that you live in South Africa.

You probably should have mentioned that along the way since almost all the alternatives to Alprostadil are probably off the table for you. USA is about the only place you can get BiMix, Trimix or Quadmix. It's like the wild west here for compounding drugs -- they allow just about anything to be produced if it can be sold.

It does sound like you are super-sensitive to PGE-1 (Alprostadil). You might also just be not used to getting a raging hard-on after all these ... months? years? The easiest way to condition a lazy couch potato penis that has not had an erection in a long while is to do a lot of pumping using a VED. That alone at first will get you up to some painful erection levels --- but of course in that case you just back off on the vacuum a bit and then go again a little -- and little by little (could take months) you will tolerate a 100% erection, With shots, well, you are stuck with your 100% painful erection.

How much more can you cut back on your shot amount? Assuming you have 20mcg/ml in the prescription that means you are doing 5mcg delivered in a .25ml shot. That is not very much but if it is giving you a good erection maybe you can cut down even more. Easy to do.

Report back on any of this ...

For all of you that I have suggested creating a signature, the points that JimStars is making is exactly what I was talking about... anyway, enough of the commercial for signatures, you may be at the point where you need to start considering implants. Just remember, you are probably looking at, at least 6 weeks AFTER surgery, before you could use it. Tell your doctor exactly what is happening and tell him that you are (or are not) open to discussing implants. Have a good weekend. :)

Hi JimStars.
Thanks for your advice. Yes being from South Africa is a challenge on different levels, both politically and sexually. You know I couldn't find a Autoject Ei anywhere in South Africa, my daughter had to get it for me in the US and posted it for me. But I can get Aprostadil and Trimix here. However it is not freely available as in the US, there is only one compounding pharmacist in CapeTown that I know of and you can only get to him via your doctor. Yes these mixtures is as scarce as earth quakes and cyclones in South Africa, which you have plenty of.

Regarding the VED, that is what I was using since my op in 2015 - the problem I'm having with the pump generated erection is that I've to use a very tight ring to keep the erection. And secondly it feels and looks like a blue police baton without a will of its own.

I will try a trimix mixture soon and will report back ...
DaVinci procedure in Oct 2015, PSA is between 0.02 and 0.03 since then. Nerves couldn't be spared as the cancer invaded the nerves as well as the neck of the bladder. 100% ED, used only a pump and constrictor rings
