Last year I went through a radical prostatectomy. After the surgery I was able to obtain an erection as before the surgery using the shots. However, later in the year it was found I had to go back and do radiation treatments. Although the shots still worked, I was maxed out on the dosage and was starting to hurt which made sex uncomfortable.
I spoke to my urologist who does the implants regularly and he said I was a good candidate for the implant. Finally after dealing with the insurance company since April, I was approved and now scheduled to have it done in late August.
Here is my concern. Since it had been about six months since using the shots and having an erection, it seems my penis had shrunk or at least appears that way. Although it was the same way the last time I took a shot and it came back looking normal and would look that way four days after the shot in a non erect state before going tiny again. Kind of like if you don't use it, you lose it.
I guess what I am trying to ask is should I do anything prior to the surgery to make it better? Should I use the pump to pull some blood into it and keep it looking a bit more like normal? Or should I let it be and just hope after the surgery everything will be back to normal?

The short answer is that I would definitely use the pump. A penis needs both blood flow and stretching to maintain healthy tissue, otherwise, it increases the chance of non-elastic scar tissue (plaque) replacing the elastic tissue and causing a decrease in size. That is why males get spontaneous night-time erections even before birth.
I would also use traction if I had a penile traction device. If not I would use some manual stretching and possibly apply warmth (under 105 degrees). The longer it is until your surgery the more important it is that you incorporate these measures. Most any competent doctor dealing with male sexual functioning will tell you that there is a "use it or lose it" principle in effect with the penis.
PS: The concept is to pump, hold for a minute or so and release and repeat for 10 minutes a few times a day. Do not use a constriction band unless you are doing so for sex.

3 weeks+ of 15 min VED x 2/Day study showed significant gains in length before implant surgery.

This is not a reply to any post on this forum, but I will be watching comments as I am tentatively scheduled for November 2nd, with an AMS implant. So I begin the journey.
My uro said to pump couple times a week, to keep blood flowing but doesn't feel that it would increase length. She said to pull on penis in shower and that would likely be final length.

80; married 57 years; HBP well controlled, overweight. Prostate Arterial Embolization Jan 2017: No indication of cancer. Injections failed. Implanted Nov. 2, 2018. Dr. Karin Dolan, Billings, Mt. AMS LGX 15 cm+2 2.5 cm RTE. 65 ml reservoir.

SHE is wrong. My doctor and another colleague co-authored a study on VED and length and found it initially to be beneficial after implant but has since discovered that it is good both before and after. He instructs patients to pump for 5 minutes at least 3 times a day for 2 months before. There are other doctors of Frank Talk members who also support this theory. It seems to be working for me.

I have no doubt that vacuum pumps help guys get the maximum size they are able too. However, I want to share my case where I did not pump. If I recall correctly, buy the time I started reading posts here about pumping I was close to my surgery date and didn’t do it.
My lifelong size from 16 on had been 5” flaccid and 6.5” erect. As I aged, the affects of ED set in, and especially after having a Prostatectomy, I was down to 2” at best when flaccid and often fully retracted and 4” erect. It had been 4.5”, but Peyronies started and I lost an additional half inch. My stretched flaccid length remained 4.5” and my doctor said that would’ve about what I would end up with,
During surgery my doctor got the plaque out from the Peyronies and apparently that made my penis much more flexible. After surgery I unwrapped the gauze bandaging when I was supposed to. I had been left partially inflated and it measured 4.5”. I was happy to know I had not gotten any smaller and figured that was my new size. As my body healed and I started using the pump, I could only get a few pumps in before the bulb went hard. As the pump loosened up, I could get more and more pumps in. After a few months I was back to my original 5” length when deflated and right at 6” fully pumped.
I decided after 8 months to stop measuring to see if my LGX had expanded at all like they have the potential to do. I am going to wait for my one year anniversary to see if I have had any additional growth. It looks and feels like I have gotten a bit larger. Some guys have said it takes a year or two to expand and some guys don’t get bigger. It depends on our penile tissues ability to stretch.
My point is I did not pump, but still got back the length I had lost. Best wishes to you on recovering what you lost.

I am 64 and had ED from a VL. Implanted by Dr. Ronald Anglade in Atlanta on 9/18/17. I have an AMS700LGX 21 cm via a Penoscrotal incision. Very happy with results. 6" soft and 6 3/4” x 5 5/8” hard.
