I'm new to this. Divorced male, 40 years old. Have ED due to the use of Finasteride 15 years ago. On cialis but it doesn't give as much as i would see.

Welcome to FT. I live in the states and am 62 so the only question on your list I can answer is about a wife knowing. There are guys here who have one night stands and pump up ahead of time and the woman never knew about the implant. If you have been in a relationship and the woman knew what your natural erections were like, I would say she would notice some difference. For most guys who suffer from ED, their partners are simply happy their guy is back in the game with a hard dick. I had ED for 30 years and my saint of a wife stood by me. I love my penile implant, and so does my wife. I can echo what the other guys have posted.
