It would seem to me that a continual "leak" in the system would eventually drain the circuit [ reservoir/cylinders/tubes/bulb ]. It would the stop functioning or get continually worse.
I'd be more inclined to think it to be, 1. Too small a resevoir for the size of the implant. 2. Not iinitially nstalled with sufficient volume of Saline for the system. 3. A faulty bulb/pump/valve .
