Tony’s excision, grafting and implant for Peyronies diary

Postby Tsanchez12369 » Sun Sep 16, 2018 4:52 pm

It’s been 12 hours since my excision, grafting (human cadever tissue) and implant. Doc is Dr Edward Karpman in Mountain View, surgery at El Camino Hospital, LOS Gatos CA. AMS 700 CX infrapubic 18 cm + 3 cm RTE. He inflated 100% overnight...looks straight. In hospital until tomorrow am when he removes the catheter and deflates to 70% for 10 days when I return to office for deflate and begin home cycling. Needed two doses of oral pain meds so far but I had the degloving, excision and grafting as well as the implant install.

Day 2 am, feeling better waiting for doc to check in & remove catheter. Feels black and blue and swollen, took a pain pill in advance of removal-hope I timed it right.

PM day 2, For me the catheter removal was brutal, and that was with pain Rx. But it’s out, bandage is off and deflated to 70% for 10 days when I return to doc. Have to keep the suture line (from excision and grafting) with antibiotic ointment and massage pump (prevent adhesions) each time after I urinate. Told to keep penis out whenever possible and if i have to leave the house to wear it up. Pain tolerable, swollen but to be expected. I’m sure it all will be well worth it! He said Friday it’ll be at it’s worse and then return to normal thereafter. Penis had 72 hr pain block which wears off in 32 hrs...

Day 3, swelling was worse last night but better this morning after icing.

The reason I went for the implant was that I had exhausted all peyronies treatments after one year and curve had decreased only from 90 to 60 degrees. Even worse was the wasting/hourglassing. The pills and cockrings no longer worked for ED but injections did. I was willing to try the injections again even though it gave me peyronies if the peyronies treatment had worked better. I was told I hadn’t compressed the injection site long enough after injection to stop the internal bleeding. Given the severe hourglassing and 60 degree curve Dr Karpman recommended the excision and grafting to return the penis to its normal state w the implant for ED and to maintain it.

Noon, took my first shower. Just one spot of slight bleeding at degloving incision. Realized I had only been icing the infrapubic area. Penis is very swollen, I’m going to ice it as well.

Have to say I’m a bit surprised of the amount/size of hardware in my scrotum.

Started Vit K, Bromelain, and Arnica 3 days before surgery and continuing in hopes of decreasing bruising and swelling and to prevent hematomas.

Icing brought swelling down somewhat. Pic w antibiotic cream on suture line. In pubic area can see infrapubic incision site and drain hole area.

Day 4

Woke up w pains but had skipped overnight pain pill. Took that and after icing feeling better. As doc said-looks worse today. Hoping to see decreased swelling and bruising each day hereafter. No sign of infections at sutures.

Infrapubic incision looks pretty knarly-hopefully just swelling. Anyone have that and it improved?

2nd pic below

Day 5, swelling and bruising basically unchanged. (Am-first pic below)

Will try hot steamroom (don’t have tub) as recommended to see if that helps any. Was able to take a short drive today w/o any problem.

Heat seemed to reduce pain but increase swelling. Swelling may also have worsened to a trip outside wearing it up. Icing now to decrease swelling.

Day 6:

Swelling bruising unchanged. Pain somewhat less.

Penis seems numb—hoping degloving, excision and grafting did not cause any irreversible nerve damage.

Trying hothands since no access to soaking tub.

Adding pre-peyronies photo and pre-surgery photo w peyronies.

Day 7:

Pain very tolerable now, only on ibuprofen and Tylenol.

Color seems to be returning to normal.

Still numb from circumcision suture line to tip. Anyone have numbness after excision and grafting?

Hi Tony, I've been following your posts over on the Peyronie's forum. As someone who is on the waiting list for exactly the same, graft/reconstruct and implant with Titan surgery, can I ask how long the procedure took? My research (doesn't appear to be much from folk who've had this triple willy whammy) so far has thrown up lots of variables, from 3 hours to 7 hours total. I've not yet had a chance to ask my own surgeon, but it would be good to have some recent real world input. Thanks!

It took about two hours. With the numbness I’m having if it doesn’t improve I’m wishing I had just gone w the Titan implant for as much improvement to the curve as possible without risking damaging the nerve bundle due to excision and grafting.

Tony,

I’m 2 years post op now and went with the implant for the exact reason you did. My peyronies was about 70 degrees with accompanying ED. Dr. Lue preformed my surgery and used the AMX700 CX, same as you.

I was questioning my choices immediately after surgery also but I can tell you that I am now throughly pleased with how everything turned out.

Your pictures, to me, look great. Allow everything to heal and begin cycling as soon as possible and I’ll bet you’ll be completely happy. The feeling will return, im sure. Mine did!

Thanks Jim! So you had initial numbness? How long before it went away? It may be wishful thinking, but a little feeling may be returning...

Day 9:

Bleeding from incision site when changed gauze wrap (over antibiotic).

Think color is more normal and swelling done a bit,

Some feeling may be returning to glans, fingers crossed.

Follow up w MD tomorrow for deflation and begin 2-3 x’s/day cycling.

Keep posting your healing and best of luck to you.

Day 10:

Dr Karpman showed me how to deflate and inflate-quite easy and didn’t take long to get the hang of it.

He suggests the numbness is do to a stretch injury to the nerve bundle and should resolve soon-no longer than 3 months.

He showed me pics of the operation. With the implant alone I was only down to 30 degrees so he proceeded w the rxcision which I believe was 5 x 3 cm.

Recommends as much time pumped up as possible to maximize gains,

Before surgery my bone pressed stretch test was 7.25”. Full inflation bone pressed erection is 6”-so hoping some gains continue although he indicated to watch for gains in girth.

Angle of erection is right at 4 o’clock, happy with this but wearing up to possibly improve it.
