You are in the danger zone. I only had ED became totally impotent for two years went from 6" to 5 3/8" It took about 15 years before that to go from 7" to 6". Your situation will cause a much faster and more permanent loss. I cannot believe your Doc did not put you on VED immediately after your surgery. It took me about 65 days to stretch from 5 3/8" to 7.1" using an aggressive vacuum therapy. Do yourself a HUGE favor and at least go online buy a cheap $30 device and try it for a month before you schedule your surgery. If you have gained an inch back it will be well worth it if not $30 shot. Had I gone in with 5 3/8 that would have been my very best length for life.
But likely I would have only gotten 5 1/4".
As for implants many Doctors like to chose for you after they look inside. Some will discuss it in advance many will not. Here is the truth. Coloplast and AMS are the most used. Coloplast has thicker cylinders but bigger girth sizes and far more length sizes. AMS has thinner cylinders with much less girth less options for length but they have one model the LGS that will keep expanding up to one inch longer over a period of time let's say a year or two for max length. That of course all depends on you. More frequent cycling faster stretching. If you don't use it much then it may stay the same length.
You can chose your own implant. No Doctor can legally deny you that right in the states. I chose the LGX simply because I do not trust any Doctor and wanted to make damn sure I could recover if he shorted me. Well I was 6.5" the night before surgery and right now I am consistently hitting 6 1/4". I'm only a little over 70 days after implant and with the shrink to fit method that all American surgeons use (a rare few will pump you up before you go home) that is a great recovery rate.
You can ask your Doctor to pump you 5 pumps before you leave the center. He might but as soon as the bandages come off and the soreness goes down you need to flaccid stretch ( grab the head of your penis and gently pull) or it will hurt like hell and you will daily curse me for this advice. But your length will come back much faster in many cases almost immediately.
If you don't ask then you will likely leave the center will a very little nub that will scar over within 2 weeks and then 6 weeks after surgery you will need to pump like hell and really work those tips (ouch ) to break the scarring and re stretch yourself to your desired or potential length.
Let me say right here that guys who have lost length like you and did not regain it before implant are still struggling to. I was pumped 2 pumps before I left but I lost a month from some stupid surgical errors or I would already have reached 6.5.
If you shrink to fit and are not pumped Coloplast being thicker will not allow you penis to shrink as small and should recover much quicker if you really cycle.
I'm having a real time getting use to the softer (by no means soft) AMS product. I'm sure Coloplast Titan might even be worse for me but you might adapt just as quickly to either one. No way to tell.
Good luck and chose your surgeon well. And oh yes you might tell your surgeon that you plan to use a mild VED therapy after implant to help grow back your girth by replenishing your dilated cylinders with blood on a daily bases. This will also keep your glans healthy because your lack of erections can no longer do that.

I think everyone is different, it just depends on your body. I think it's a good idea to read all the info you can from this board and anywhere else. Sift through all the info and make your own decision. I believe a good Dr. is important. I live in a state that has only one Dr. that can do this so I chose him. He has done way over a thousand implant surgery's in the last 20 years. Turned out that he did a very good job on mine, at least to this point in time.
My story is quite different from a lot of the other guys posting here. I'm about 7 mos. post op as I write this. My DR. sent me home with a full hard on for one week. Went back for my first checkup after one week. He deflated it and it stayed that way til the 6th week. I went back in and he showed me how to inflate and deflate, he did it once and I did it a couple times and he said your good to go. Said it's a good idea to inflate it once a day and hold for one hour, which I did faithfully till the forth month. I then e-mailed him and asked how long am I supposed to continue cycling as it was hurting all the time. He told me to stop cycling and just use it like normal which I have done. I still have a little tenderness in the shaft, but it works better that my old dick did for the last 15 years.
I lost no length from the surgery and I gained a little girth. I love the bulge in my pants as I was never a shower. It was only about 3" before the surgery now the flaccid length is right at 6".
I no longer have to hunt for it in front of the urinal. Don't dribble on my pants anymore either. And my Medi-care paid it all except a small co-pay. Life is good.

Re: I need advice
I'm 67 now, 66 when I got my implant. I've mentioned on other threads here that I wished I had the implant much sooner. I struggled along with oral meds first, they worked for some years but gradually lost efficacy. Then they issued me a pump. That didn't work for me very well at all and they quickly went to the injections of tri-mix. This worked wonders for a couple of years and then gradually began losing efficacy. Additionally, I started noticing a shrinking of Willy and some scar tissue forming on the right side, which caused him to veer to the left. That got worse. Along about then I transitioned from my group insurance at work to the VA system when I retired from my career. As a Cat 3 disabled vet, I have always been qualified for free med care but for anything beyond the local clinic, have to travel to get it. At my first appointment with the urology clinic they were matter of fact that they felt I needed an implant. In discussing it I thought it might be better to continue with the injections supplemented by 1000mg Viagra. This combo set me up for success for about two more years and they were willing to do that if its what I wanted. Finally it got to the point that even that wasn't doing the job plus I was getting smaller, and forming more scar tissue (I've been on Warfarin for 17 years due to a clot and embolism apparently caused by a certain protein deficiency, and this probably contributed to having small hematomas at the injection sites which formed permanent scarring). So I set it up and had my implant about 15 months ago now. I wished I had done it sooner! I started out post op not being able to even find the pump due to a giant hematoma nearly filling my scrotum up. It took about two weeks before the surgeon could get to the pump and begin manipulating it. He had left me about five pumps full post op, thank goodness. Anyway all that gradually cleared and I started pumping up and down almost every day to try to avoid scar tissue, on their advice. I would say that I was the same length or maybe just a tad shorter post op than before, but since I had lost nearly 1.5 inches before surgery, I was despairing a bit. It seemed to work ok for both intercourse and masturbation so I figured I'd live with it, a nice trade for never having to worry about having an erection. Also my orgasms were and continue to be the most intense I've had in my life, I'm not sure why that happened but I'm not complaining. Finally, in the last three months or so I realized that I was feeling larger, both length wise and girth. I did a little measuring and I'm now about 1/2" longer than pre-op. So I'm just getting happier and happier. So my recommendation always now is, if you've done enough to get your insurance to pay for it, don't be apprehensive about the surgery. It was more a pain in the ass than actually painful for me, it works great, I'm actually getting bigger, and it feels great to use...win win win etc.
Surgery at VA July 2017 after many years struggling with pills, shots, pump, etc. Should've done it way sooner! October 2020: VA just replaced the first one with another due to stuck valve...AMS 700 LGX.

williamh8, try reading this thread for ideas. There are differences between the 2 main brands and variations of each. I do know a couple doctors who suggest this as reading material for patients. Cheers man. viewtopic.php?f=6&t=8032&hilit=the+imperfect+implant

Re: I need advice
About which brand. I find that most Uros have already decided that. Unless you make a fuss they will choose for you. Find a Uro and discuss it before hand.
I have an AMS and really you do not need to worry about hardness. Three pumps is more than enough to do the job. I never fully pump and many guys are the same simply because of the discomfort of tips when fully pumped.
For you to personally choose you need to have some very basic preference like I did. Otherwise you are much better served letting your doc open you up and choose from what he finds.
Titan has much greater choice in lengths and I think in girths. More lengths means less needs for TREs rear tip extenders. Usually that is not an issue. 2 or 3 with either product is best. less chance of erosion.. I do know that Titan offers a much larger girth. I was stretched out to 20 mm should have had a Titan to fill that giant void not the 12 mm offered by AMS. If you want bigger faster girth then Titan is your choice. Unless you only have 5" girth now, AMS handles that easily. As for hardness not an issue. When deflated the Titan feels like plastic tubes inside of you. The AMS is softer but by no means soft. Like OldBeek I am having a very hard time adjusting to the hard feeling inside of me. I cannot imagine what a Titan would feel like. Finally I did not trust my uros to give me even the basic smaller penis that they promised. So I went with the AMS LGX because in a year or two I could gain an extra inch that was stolen by my Doc. Turns out there was no need. I'm longer at 70+ days than I really wanted to be. So that's just me.
My hang is too long but is lower and more natural than a Titan would at this point give. So I don't need to bend myself down into place. Just deflate fully and let go. The cylinders seem to refill or reround and then just hang down. Titan are stiffer, less natural at first, may soften eventually.
I hear that the Titan pump is easier to use but has a tendency to auto fill sometimes. The AMS pump has a very hard start is easily locked up and difficult to unlock but never back fills.
Last thing Titan will recover length faster with less help than AMS. Because they are thicker. May be a huge plus for you. Me I used VED to do that job much safer, faster and more comfortable than having the tips do it.
Bottom line unless you actually have a firm reason and call either Coloplast or AMS to have an RN rep at your surgery then your uro might just choose for you.
Good question to ask right up front!
Good luck!

Re: I need advice
The coloplast gets harder.
Sorry ED2013, but I disagree.
That "harder in the hard state" video is a fallacy that's been circulated for a long time here and other places. For me, anyway. The AMS gets just as hard as the Titan. It's dependent on just how hard you pump it up. The inflated state is somewhat different. My Titan had stiffer cylinders - not harder, just different. My wife is much happier with my AMS because she says it looks and feels more like the real original.
Before my revision, my surgeon left me alone with actual fully functional working models of the Titan and the AMS LGX. I recommend that anyone considering what they want should play with these type models. There are significant differences. If your surgeon doesn't have them, ask him to get them. If he won't, look for another surgeon. It's a life-altering decision and you shouldn't go into based on blind trust. No insult to the medical profession is intended. My wife is a physician and my son and daughter are both physicans.
There are some situations/reasons why the Titan is better. For example - large dicks and Peyronie's curvature. The Titan took care of my Peyronies. And I liked the feel of the Titan pump much better. But, for me, and just IMO, the AMS is the better implant.

I'm guessing you meant 30 pumps.

Re: I need advice
Unlike a number of you my size did not come back. I am two years out and still at 4" an inch less than before e.d. Initially I was upset and I still think my penis would accommodate longer implant chambers However wife and I have just had to get creative with different positions that work with my smaller equipment.
Age 68 - AMS LGX 12+2cm. Lost over an inch from pre-ED length but thicker for what it's worth (not much). ED result of heart meds.
