Quad Mix cost questions

I got 5 ml of quadmix (which at my dosage is about 200 doses) for $120 from my local compounding pharmacy - but I only paid to $40 due to my insurance co-pay. The first batch lasted nearly 2 years in the fridge. It was a bit weird -- it basically suddenly stopped working, not some kind of slow degradation.
