For quite some time in the few years after full erections became impossible or extremely short-lived, semi-soft orgasms (fellatio or masturbation) were the only ones I had.
Lack of erection does not turn the nerves off, nor the physiological response (of orgasm) to stimulation. Erection under the nerves/skin may make the sensations more sensational, but do not stop them!

Whether or not guys can "get off while flaccid" often depends on matters of anatomy: size, looseness of shaft skin, if a foreskin is present, etc. (I can imagine how frustrating it could be.)
