As a further clarification, I think (but someone can correct me if I'm wrong), the cylinders always stay within the corpora cavernosa (when placed properly). It is the tips of the cavernosa that "extend into the glans", along with the tip of the cylinders. I would understand the relationship between the glans and the cavernosa to be analagous to a mushroom cap (glans) and mushroom stalk (cavernosa). If the tips of the cylinders literally break out of the cavernosa and into the glans (eg an "erosion") this is actually quite serious problem that will likely lead to a quick trip to the operating room.
