The nerve issue makes sense now that I’ve heard what you experience. And the Doppler ruled out venous leak or other organic issues.

Diagnosed venous leak. Going with injections rather than implant for the time being.

I've been struggling with ED since 2019. Mine was caused by damage to the penis, so it happened overnight. I had a few failures using 100mg Sildenafil so I went to a urologist to see what the next options were, and that's when I was prescribed injections. I was terrified at first but there are actually some plus sides to using them...
1) Don't have to take them on an empty stomach
2) don't have to wait an hour for them to take effect
3) it has reduced my performance anxiety knowing that I have injections to hand if Sildenafil doesn't work again.
I think my partner prefers the injections... If we suddenly get in the mood, a quick injection and we're off and away - no need to plan it and wait an hour.
British, 32 yrs old. ED caused by Paraphimosis in 2019. 100mg Sildenafil for sex. Trialled Invicorp as advised by doctor in 2021 but it was too strong. Taking 50mg-75mg of Sildenafil every night at 3am to stimulate morning erections, good response so far.

If your ED was caused by paraphimosis why don’t you get circumcised? I’m surprised a urologist did not suggest! Just wondering.

Seeing a professional therapist for the issue would be even better. I fail to see a down side to trying the treatment, certainly before engaging in more invasive treatment. I'm certainly no medical professional so my advice isn't worth much but to me you seem fixated on the nerve damage issue. I think it might be advisable to open you mind to some other diagnoses and solutions.
The Frank Talk Forum administrator seems fairly knowledgeable on this topic. You might consider soliciting his input on the topic.

I researched quite a bit on the types of CPPS and I can definitely relate to the symptoms and could imagine that I could have some sort of it. That article is really interesting, thanks for sharing. It's just a pity that the whole topic is still very much unknown in the urological field. I'm quite open to all kinds if solutions and diagnosis, yet it just seems there's no really obviously effective ones. Anyways still I'm sure that it's related to nerve issues, since also this article mentions "Prolonged contraction of the muscles results in pelvic myoneuropathy".. that's definitely something I could totally image to be true.
The administrator of Frank Talk, Paul, actually introduced me to this topic and sent me a bunch of links via mail a while ago, which I really appreciated. I've been trying some of the recommended floor excersises, but with no success. But maybe I should look even more into it. I think I don't have his mail address anymore though.. do you know how I can contact him?

I wish I had got circumcised when I was younger, it would've solved so many problems. My urologist treated me with a steroid cream which loosened the skin over time to prevent it happening again.

My erection lasts for about an hour, so a reasonable amount of time. Haven't gone for round two yet but I think I could if we wanted to keep going a little longer. For the record, I'm trying to use Sildenafil 100mg for now. Just knowing that the Invicorp is in the fridge if needed has helped to reduce my performance anxiety and has helped me have more success with viagra after a number of failures last year.

I too have suspected shrinkage over the last years. I used to have a strong 5.5" flaccid and a strong 6" erect. My Wife a few years ago decided to take a vacation from Sex and told me if I found it somewhere else, I would wind up like Mr. Bobbitt with no length. So I've dealt with the E.D. and the Shrinkage. My Wife has now decided to come out and play and I can't get in the Game. My Female Urologist wouldn't remark about shrinkage and was against an implant due to the Hospital being connected with the Catholic Church, her hands were tied, after that she was replaced with 3 other males in succession. I have now found a new Urologist who is also a Surgeon at a new local hospital and is very open to doing implants and many other things. My Female Urologist was a little afraid of touching my penis but didn't have a problem doing a vigorous rectal exam. Figure that out?
I'd enjoy some discussion about what I should do? I'm scared to death of surgery and of the future. I'm 67 years old and don't want to go and just sit. There are just too many choices.

There is considerable evidence that penile size can be maintained (even with E.D. or complete impotence) by the use of a Vacuum Erection Device (VED) to simulate/replace the normal daily/nightly erections that fully functional penises experience. VED use (without constriction rings, but just to get the penis flushed with fresh oxygenated blood regularly and tissues stretched to maintain elasticity) is important for men who do not get erections regularly by the usual means (nature).
Even the cheap sex-shop/novelty-store devices can do the job as well as more expensive medical grade ones available by prescription. So, if a urologist won't get you one, get one yourself is my recommendation.
Relationship with your wife is EXTREMELY important. Did she tell you why she took the vacation? Did she tell you why she is willing to come back? Is she on board with the idea of an implant? Have you tried less invasive treatments like oral medications? They can do wonders. Injections, even more so. VEDs also make sex possible when combined with constriction rings.
Relationship with your medical care team is also extremely important. I am one who believes in being a very participatory patient.
