I always leave three or four pumps to avoid a MS valve problem sticking. I have had two revisions after the original, and the last two it was because of the MS pump valve getting stuck to where the ball which you squeeze would stay collapsed and it was panic time. But if you leave some saline pumped in the cylinders, I have not had that problem.

Now to answer about what happens to the gland when your penis has stretched over the years. In my situation the gland still got engorged when I got excited in having sex. So I guess I still had blood flowing into the gland. But in my last surgery they put longer LGX cylinders and some rte's to fill the shortage inside the penis. I got full length now and I am a very happy person.
