Here’s a few points:

Tadalafil is used up to 40mg per day for pulmonary hypertension. It must be generally recognised as safe at that dose in order for it to be prescribed at that dose. This isn’t a recommendation to take 40mg per day, but do with the information what you will.

Sildenafil and tadalafil have slightly different pharmacologies. This means you may benefit from taking both simultaneously at half the usual dosages each. The same is true for the other pde5is.

Generally when it comes to treating any pathology, combination therapies work best. For example, hypertension is best treated with multiple low-dose antihypertensives. GLP-1 agonists are being iterated upon to provide more diverse pharmacologies. And even ICC injections are designed to have multiple mechanisms of actions, with bi, tri, and quadmix existing.

Phentolamine, found in bi, tri, and quadmix is an alpha adrenergic antagonist. If you’re unable or unwilling to use bi, tri, or quadmix you could consider taking an oral alpha-1 and alpha-2 adrenergic antagonist. You can easily get doxazosin (alpha-1 antagonist) in the UK from lots of online pharmacies. Yohimbine (alpha-2 blocker) can be bought from Europe or America very cheaply and shipped to the UK. There are studies on both drugs showing they’re effective adjunctive treatments.

Pt-141 is discussed often here. It’s fairly easy and very cheap to get from Chinese sellers, but it requires a little bit of work buying crypto, and you have to “reconstitute” the peptide yourself. In the UK you can get free needles, and cheap sterile water and benzyl alcohol to mix it yourself. Pt-141 does work very well. You can even take mt-2 if you want the same effects of pt-141 with the added bonus of a gnarly tan. For reference, a 2mg dose of pt-141 or mt-2 costs me about £0.40. To my understanding, pt-141 and mt-2 are legal to import to the UK. Just look at how many women are importing glp-1 agonists, and how 0 of them have been arrested for it (assuming they’re not reselling). I’m not a legal expert, however. Do your own research on the law.
