I don't get the BC requirement that
A surgically implanted penile prosthesis may be considered MEDICALLY NECESSARY for treatment of erectile dysfunction when all of the following criteria are met
I mean geesus -- renal failure?
My BC advantage plan covered it entirely without nearly all of those requirements.
If you can get coverage, my opinion fwiw is to get the implant from a high volume surgeon. When you have to get replacements sounds like BC covers those.

Now 29. 27y/o when implanted. For 9 yrs was told it was PerfAnxiety. Tried almost everything imaginable. Diagnosis: Low penis inflow via Doppler. 9/20/22 Dr. Kohler RochesterMayo. Titan 22cm no RTE 125cc. Pre Implant length:6 Girth:idk. Post: L:6 G:4.6

I think you also need to try different pills. It looks like sildenafil has been a hit or a miss for you - it was the same with me, but Cialis/tadalafil worked much better for me. So try each combination, with a girl if possible.
33 y/o, suffering from ED as long as I can remember. Never had random boners. Pills worked for a few years, but now losing efficacy. Venous leak confirmed by Dr. Garber. Likely going the implant route at some point.
