Sambalamba: Just consider this as your anatomical feature that causes DE's. It is highly likely that if your penis will fold over & hang, you'll get a dog ear. Its a round tube, bending it will cause a dog ear. Go bend an empty toilet paper tube, you'll see the dog ear. No real way to stop it from forming in my opinion. The real answer is how do you deal with them.
As I've said with my CX. It hurt the first time & gradually went to no pain over a few months. Maybe only two months. It wasn't important enough to me to note the date.
Its my personal opinion that "if" you potential might need a 24cm CX. Your dr would of give the BS rep a notice to bring some. They are pros, they just normally carry the most common sizes to surgery. Its not they go make one while you're on the table.
As for the RTE's. I think its unlikely that they'd tinkertoy one up for you out of RTE's. RTE's are used to make size increments other than 18 or 21. In a few cases they have to move the tubing exit from the implant to match your body. But I think at least BS has two styles of the exit in the base now. So yes, you might end up with a 21 CX or up to a 23.5cm CX. All valid uses of the RTE's. If your measured as being long. There will likely be enough implant in your pelvis that it won't wobble. Just make sure you keep it pointing up to let the capsule form around the base.
I had preferred an LGX. My dr won't use 3cm of RTE's. So once I measured at 21cm. I got a CX. I'm happy with it. Works great every time. My drs nurse says he seldom does a revision on an implant he installed. But does a fair number of revisions on implants from other drs.

That makes sense, GT1956. So, for those with a length of 5 or 6 inches or more, is it almost inevitable that dog ears will occur? With that length, it seems likely that the penis would fold over when flaccid, increasing the chances of dog ears. Also, do you think that having more girth—or more remaining corporal tissue—would help minimize the severity of the folds? It seems like the extra tissue could act as a buffer, softening the kink and making the folds less pronounced. I wonder if a balance between length and girth plays a role in how noticeable the dog ears might be.
55 years. Using bimix 0.4 units. Works well but inconsistent and very inconvenient. Seriously considering an implant.

I can't snswer the girth vs dog ears question. I always had this girth. I still had most of it after my sugery. Somewhere around 3 months I was back up to 6". I never measured girth pre ED. But just a guess on how my hands wrap around it. I'll guess that I lost a little. Not enough to tell without data to compare. My wife was great at feeling my girth. She never said I lost any. So I defer my opinion to the expert in the family.
