12 years, since my erections just stopped when I was 47, tried every possible treatment: different pills, Injections, pessaries, VED , none of which worked in the desired manner, but were mostly unpleasant in some way. Injections, very painful and ineffective, VED; tried several different cylinders but in each case, my penis expanded to fill it and I applied the cock ring but on removing the cylinder, my penis was still flaccid, just a lot bigger, pills were the worse, taking under medical supervision, no effect at all, until an increased dose caused a very unpleasant reaction involving a temperature spike of 40°C and temporary blindness, after which I was told never to take Viagara again, Cialis, no effect, Pessaries, no effect.
Eventually became eligible for implantation with an IPP, which took place in in April 2023, op and recovery went extremely well, after which it turned out that my prosthesis was faulty and would not operate, not even once.
Last week I had that faulty IPP removed and another implanted, again all has gone well, but I am trying not to get too excited about it, but the partial inflation I had upon waking up was the closest thing I have had to a boner in all these years.

Onset of ED at age 46, no erection since, pills, VED , Injections, no luck, not even once.
Implanted 19.04.2023 in UK, with Rigicon Infla 10 AX, not functioning.
Had revision 24/09/25 Rigicon removed and AMS700 implanted.

Coloplast titan 26 cm placed on 8/26/25 by Dr Taj
~8" before IPP but not "hard" enough for pvi.
Tried everything as things progressed:
Pills, pumps, prp, trt, gainswave, peptides, trimix, quadmix and a vibrating ring.

Same... and let me give you a little hope:
Im 51 and I'd say 90-95% of the sex I've had in my life has come with anxiety, stress, underperformance and apologies. At first I was always concerned about finishing too fast (and pretty much always did). For a BRIEF time with my now wife, gf at the time I had amazing confident sex (when it was daily. While we were early dating). When the frequency dropped i went back to finishing too quick. Somewhere between 35 and 40 it flipped to struggling to get and keep an erection.
Very frustrating. Also definitely made sex stressful and disappointing for my wife. It felt like a test for me and a chore for her.
Well, I got implanted in August and had sex once (cleared at 3 weeks, had sex, pulled my incision opened, no longer cleared, just got cleared again!).
Let me tell you: it is a whole new experience. So crazy to walk to the bed rock hard without rushing (worried about losing whatever erection I had). Letting her handle me without worrying about if it'll be soft or usable... and being able to take my time and get her VERY ready without losing my erection. Being able to calmly go at for as long as she wants and being able to watch and hear and feel her cum hard before finishing myself... and the look of shock and satisfaction on her face afterwards made every bit of this 10000% worth it.
This (at least for me) has been the real thing. The fix i was searching for when I first found pills. The solution i thought I had when I first tried trimix. The answer i hoped gainswave and the pShot would be... but this IS the real thing. I just got cleared again so I guess time will tell but I read what you wrote and related. This is like the first time except totally different in all the good ways.
I hope your IPP journey is as positive as mine has been (even with the bumps along the way).
That’s awesome Colby, I’m really happy for you and your wife and hope your IPP keeps delivering for years and years to come!
I had my 3rd consultation with Hakky today and we’re proceeding with the implant path. Now I need to determine if insurance will cover it or if I need to go out of pocket. I’m leaning in the direction of starting with an IPP and switching to MPP if I run into an early mechanical failure, but haven’t made my final decision yet. I suspect insurance coverage will play a major part in that decision.
Very excited to start my journey and join the crew.
Diagnosed with venous leak after having ED majority of my life. Grateful to have some answers, and considering both MPP and IPP as options.

JohnnyBorg had hakky something to say about this potential strategy? (First ipp then mpp).
Hard flaccid syndrome since 2019. Trying to get better with conservative treatments but an implant is on my radar

JohnnyBorg had hakky something to say about this potential strategy? (First ipp then mpp).
Yes! This will be a longer reply so stick with me. I actually plan on starting a dedicated thread related to my pre-op journey (and eventually one for my implant journey) and I want to include the details of what he and I talked about today, was a GREAT conversation.
But the short version: he did believe this is a solid strategy - because if you’re able to cycle the IPP and stretch the tissue you’ll have the capsule form, and if you switch to an MPP later you’ll have a much better sense of your potential MPP length and girth. It will still be slightly less than where you’re at with the IPP (Hakky usually downsizes length by 1 cm for healthy patients and 2 cm for patients with diabetes / immunocompromised patients to help prevent erosion. Girth will be dependent on your static MPP rod size, so you might lose a little there too) but if you’ve expanded the tissue out with an IPP and gained back some size, you mitigate some of the overall potential size loss.
I hope my explanation makes sense, I’m very much paraphrasing what he said. The way I’ve understood it holistically, here’s the TLDR version:
If you are looking for the absolute lowest risk and the most likely MINIMUM number of lifetime surgeries, go with an MPP - but understand the potential trade offs. Some guys don’t lose any size, some do. Some guys don’t mind the discomfort of the constantly static erect rods, some do.
If you are looking for a device with true flaccid state, and are working with a high volume surgeon that knows their craft and understands the potential revisions over your lifetime, go with the IPP. It’s the most surefire way to get the most out of your size (I know some guys do lose some size with an IPP and Hakky told me that as well, but the mechanism of cycling and inflating is your best bet at regaining or retaining it). It gets rigid as hell. It does have a higher risk of mechanical failure and more potential revisions - but Hakky actually did more of a deep dive on this with me and had some good things to say about it which I’ll also share.
And finally, a “hybrid” approach could be to start with an IPP, see how far it gets you, and then when it fails (whether that’s in 8 months or 8 years or more) revisit the IPP vs MPP strategy and see if switching to an MPP is something you’d ultimately prefer. You’ll gain the benefit of cycling + the capsule forming around the implant, but you’re not stuck in the IPP ecosystem. You absolutely can make the switch to MPP especially if you have an unfortunate early failure and don’t want to do a second IPP attempt. This could be a good move for younger guys who have many years of sex (and therefore potential revisions) ahead of them, in order to reduce the total number of lifetime revisions. Maybe you go with the IPP and if you get 8 or more years out of it, AMAZING. If not? And if you don’t want to attempt a second IPP? Make the switch to an MPP. Your overall lifetime # of surgeries would still likely be low overall in that case.
I’ll end by saying again what a great dude Hakky is. He really took his time today talking with me about all the options and his candid opinions about them. PowerliftingDad already said it best in his recent thread - do your due diligence / research, and pick a high volume surgeon.
Cheers everyone. I’m riding a bit of a high today after that consult with Hakky. I’ve got a long road ahead but I’m all for it.

I can’t thank you enough for this post. This is truly valuable informations for all young people considering an implant. And i'm curious to know more about the part where you said "It does have a higher risk of mechanical failure and more potential revisions - but Hakky actually did more of a deep dive on this with me and had some good things to say about it which I’ll also share"

I take an oral pill version of the main ingredient called Rybesus. I bought the largest pill available, 14mg. My plan was to cut the pills up as I slowly increased my dose. 1/4's were too hard to reliably cut. 1/3's were easier to manage. I'm now on 1/2's. I started at 245lbs. The weight loss started quickly. I'm now 215lbs but it can be rough at times. Some bloating, diarrhea nearly every day. Never trust a fart. It doesn't absorb in the intestine very well. They add a chemical to get your stomach to absorb it. I take it in the morning on an empty stomach with as little water as possible then I lay back down for a while. I want it to sit in my stomach as long as possible in order to get the max absorption.
I have stopped taking my fast insulin, still take the slow release insulin. In a few weeks I'll have my A1c checked to see how my diabetes is doing.
It is a crazy drug. It inhibits your appetite. Sometimes I have to actually force myself to eat. I've read that it increases your natural insulin production and/or grows new insulin producing cells. My dr approved my idea. Skipped the battle with insurance coverage. It's off patent in Canada so I paid cash. It is not cheap even in Canada, just under, $10 a pill. But how I do it it's under $5 a pill. Semaglutide is not for everyone. I read of people that stop due to the hassle. But, again just my reading. Lots of people only take it until they reach a target weight. That is my plan. Maybe cycle on & off it for my lifetime. Who knows but my main concern was the diabetes control that I can't report on as of yet. Yup this drug is the new cash cow for the pharmacy companies for a reason.

Yes, my major side effect was nausea, about once a day I feel like puking for about 10 seconds then it subsides, after 2 years it is very infrequent now but have lost 80LB from 300Lb to 220Lb.
