I used to have perfect erection in my earlier days. I have been dating my last ex for 2.5 years and everything was normal. I was in my 20s back then when I realised that I am facing OAB problems. Soon after the breakup (i was 20) I’ve realised that I couldn’t keep an erection while masturbating and ejaculated soon. I’ve stopped masturbation fearing that it might the reason.
Day by day my erection strength was decreasing but I thought it’s normal since I was single and not watching any pornography nor masturbating.
I’ve also consulted a doctor but he said that it’s psychological since my erections were normal previously rather he gave me meds for OAB only. I’ve suffered from this for 3 years thinking that it’s normal.
Last year I’ve consulted another urologist for the same problem. He did all the blood tests, hormal tests and urinal tests which were normal. He did a doppler test to check if I have venous leakage. I had to take tadalafil 20 mg 1 hr prior to the test. But the problem was when he asked me to rub my penis and make it erect, I couldn’t do it. I knew that if I try more I’ll ejaculate in the flaccid state. So he did the test on the flaccid state and found similar psv and edv value before and after the medicine.
He then concluded that I have no venous leakage but I do have some other vasculogenic ed. I don’t think the diagnosis was a proper one since there was no erection at all.
I do get random weak erections and also weak morning erections too.
After 4 years I am now dating someone amd I really don’t know what to do.
I’m only 24 with both ED and PE.
Please help me in finding the cause and suggest me a possible solution.
The doctor suggested me to maintain a diet, quit smoking and exercise. Is that going to cure my problem?

You might want to consider testing Sildenafil. Sometimes forcing an erection helps force blood into unhealthy tissue to help revive it.

Your doctor is right and that is an excellent piece of advice: a healthy lifestyle is the #1 ED treatment.
Your post is a bit vague, your doctor spoke of vasculogenic ED but did he specify the cause?
Even if this is vasculogenic, ED is a multifactorial condition and there may also be psychological factors involved (e.g. anxiety, depression).
Before you try any medication, I suggest you get a second opinion.
Age 40. Psychogenic ED for over 20 years. Current regimen: Udenafil 200 mg, oral phentolamine mesylate 40 mg, Seredyn.

Hey man, I'm 25 and I have pretty much the same problem as you.
I have OAB, erectile dysfunction and additionally increased bowel movement.
I've been frustrated with this situation since the past 3.5 years and went to several doctors and urologists but noone could give me a proper diagnosis. Urologists always blamed it on a psychological/psychosomatic issue.
However, now I am pretty sure that it is caused by an autonomic neuropathy because of smoking and drinking. I never smoked and drank much but had a quite abusive intake for a short while when studying abroad and after that those symptoms hit me immediately but I never knew what it was and thought it would go away. Also the doctors said that it seems unlogical.
I researched a lot and apparently damaged nerves (around the pelvic area) can exactly causes these problems:
neurogenic bladder
sexual disfunction
neurogenic bowel
In addition, I feel a needle like pain from time to time around my left testicle area, which I thought at first was some blockage that could cause the ED but no doctor found anything. Now I'm pretty sure it's the nerve pain that I'm feeling from the neurpathy, which is similar to the neurpathy that causes ED in patients with diabetes. I'm just as frustrated as you. From social drinkink and smoking I came to totally quit everything but no improvements. I don't know what to do really, it's a course and makes dating almost impossible and super embarrassing. It's been too long already. Do you have similar nerve pain that you notice?

Can you please tell me more about your psychogenic ed?
Can you have sex?
Do you have a partner?
Do you have morning wood?

NO man I don’t have any pain. I don’t know what will happen to us. I had strong erections and now I’m suffering from it.It all started with OAB and the doctor couldn’t find a proper solution too.
Have you tried pills?

I tried all pills and dosages and I'm at a point where nothing is working anymore. What about you, have you tried other pills than tadalafil?
Maybe you don't feel the pain because your nerve damage is not that severe. But I don't know, in general it seems like we have a similar cause though.
I had really strong erections too before the ED happened all of a sudden, I feel you, I'm suffering a lot too. Relationships didn't work out the past 3.5 years, sex was really difficult for me to achieve, no fun, and only with pills somehow possible. Viagra 100mg used to make me have sex somehow which got me through my last relationship but now it's not doing anything anymore, kinda got immune. I don't get any spontaneous erections anymore either. I'm thinking about an implant right now as a last resort, but I'm really scared of it and I don't wanna destroy my penis when I know that the hardware is still all fine, it's just the software (nerves and brain) that doesn't work. But I also don't wanna waste more time in this completely powerless and depressing state. Don't know what will happen to us either

You may try injections too.
I do get erections but these are weak and doesn’t last long.
I haven’t tried any pills. I don’t know if that’ll work for me.
Being in a third world country, it’s very hard and expensive to have an implant. Even if I do that, I need to go for Indian Malleable ones which sucks too. But if I’m left with no option then that’s all I would have to do.

I suffered from psychogenic ED for many years and I barely had any sex in my 20s. My self-esteem was low and I didn't even know this was a medical condition.
When I turned 30, I started doing some research on the subject of ED and my sex life improved dramatically.
Now I know that I suffered from sexual performance anxiety (coital anxiety), which is extremely common among young men.
I also took finasteride for hair loss, which may have worsened my ED.
I consulted a urologist and started taking Cialis. I added oral phentolamine to my regimen and a couple of natural supplements (Seredyn for anxiety and PRELOX). Today I'm 43 y.o. and enjoy a great sex life.
Age 40. Psychogenic ED for over 20 years. Current regimen: Udenafil 200 mg, oral phentolamine mesylate 40 mg, Seredyn.
