It's been a long time I haven't been here. Hope everyone is doing good !
I had my AMX 700 implant beginning of 2016. I was never really active sexually since. It was always ONS. The implant is working great, every session is at least 1 hour but I always finish by masturbating while kissing. Lately, I had a girlfriend to whom I told everything about my situation. Sex is great between us, a combination of love and lust. Just one thing fucking up my mind: I cannot orgasm while having sex! I can only achieve it only while playing with myself. She is really trying to help, because she really wants me to be happy. Handjobs, blowjobs, we tried all positions. All the same.

So far in nine months I have only had one PIV orgasm but it was also the first in about 18 years too. It was glorious but it hasn't been repeated yet in several subsequent sessions.
I'm sure with time and conditioning the ability will come back more reliably. One thing others have suggested here: ABSTAIN FROM PORN AND MASTURBATION
Untraining your brain to expect your specific movements and strokes, as well as the audio/visual cues, may lead to a better chance of attaining the goal you are seeking, PIV Orgasm. You will be training your brain (and dick) to get more excited by the stimulation you DO get, specifically PIV stimulation.
I also find that I tend to "GET BUSY" more readily than before I got the implant, because I know I can without 100% physical arousal. Getting the arousal level up more before actually engaging in PIV sex may help shorten the time to release. I found that it's not necessarily impossible to cum but it just takes more time and stimulation than I'm able to provide in an average session. I haven't tried the porn/masturbation abstinence method yet but I may at some point.

I only began having vaginal orgasms 3 years after my implant surgery. I can say in all seriousness that having my GF suck me for 15 minutes leading up to fucking really helps. I can ejaculate without the tongue work but I shoot more quickly if she does.

I find that being pumped extra hard sometimes prevents me from getting off. So maybe not pump so hard might help. I also find that i get great sensation either if shes super wet or add some lube. As far as actually cumming, i have dry orgasms due to my prostate being removed. Orgasms are intense just no ejaculate. I do miss it but small price to pay for being alive. 6 years, cancer free !

My wife takes it personally that she can't make me shoot (other than once so far). She said she feels it's her fault. When I did get an in-vagina orgasm a couple of weeks back, she first only noticed I was "PULSING" or "THROBBING" until she noticed the hotness inside and then figured it out. I would re- iterate just how much easier it is to ejaculate/orgasm vaginally AFTER having my cock and balls sucked and played with. I found too that watching porn while being blown increased my sensitivity level that allowed me to orgasm more easily.
I notice that I’m several years older than you.

Smetro, I'm also borderline low-T. Probably need to get on TRT and that would go a long way to improving the situation. As recently as a 1.5 years ago, even with PD/ED I was able to express seminal fluid (cum) from my penis by pushing hard in my abdomen (like when constipated). I've been able to do that for 20 years but now it's rarely possible to get the force cum without ejaculation. Maybe partly my diet - need more H2O consumption...
I'm taking Cabergoline 0.5mg twice weekly (it reduces prolactin levels which supposedly can improve DE symptoms), and also stacking Zinc, Pygeum and Lecithin supplements. I just started again taking 5mg Cialis after stopping for several weeks without any change. With the supplement stack I take, I get plenty of pre-cum but it's still just difficult to get the autonomic mechanism triggered to get the orgasm.
I've really thought about getting an inhaled Oxytocin to see if it's all they say it is... At the time of peak arousal, take a couple of nasal hits of the Oxytocin spray and it's said to trigger an orgasm.
Think I'll talk to my Uro on my next visit (not due for a while) and see if he'll get me on Testosterone, either cream, shots, or the pellets.
