Trimix dosage
With the units marked on the 100 syringe so close together how accurate do I have to be?

How many units are you using?
If you're using 40 units being off one or 2 units probably isn't a big deal. If using 4 or 5 units being off one or 2 units might have a huge impact. Smaller syringes generally have hash marks denoting smaller measurements. Most 1/3cc (30 unit) syringes have clear marks down to 1/2 of a unit.
Are you just saying it's a U-100 insulin syringe or are you saying it's a 100 unit U-100 syringe? If you're only using 5 to 20 units then you should use a 30 unit (1/3 cc) syringe. If using 20 to 40 units then you should be using a 50 unit (1/2 cc) syringe. I would only use a 100 unit (1cc) syringe if I needed more than 40 units. The smaller the syringe the clearer and easier to read. Use the correct size of syringe.
The exception would be if using an auto injector. I don't think they will work properly with syringes smaller than 1/2cc.

The syringe is 100 units, 1cc/ml. I fill to on or around 10 units. Suppose to take 5 to 12 units. I understand this is a beginning amount and there doesn't seem to be an average since we are all different. I want to be careful but not scared.

No need to be scared but you do need smaller syringes. It's easier to tell if you're needle placement is correct with the smaller syringes due to less plunger resistance. I don't use BD brand because they have more plunger resistance, at least with smaller syringes. Remember, if the plunger is difficult to depress you need to adjust your needle either in or out.

100%! I typically am good roughly between 15-20 units and using a smaller syringe is easier to measure with the smaller dose. I switched to a 0.5ml syringe and it is much easier to keep consistent doses.

So I have a insulin syringe gos 10nl to 100 ml me first dose is supposed to be 0.2 I think that is to much to start with I would like to start with 0.05 so where I that on this syringe

I'm not a medical professional of any kind. Anything I post is based on my own experiences at best and hallucinations and/or delusions at worst. Internet forum advice is likely worth what you paid for it, or worse. Always ask your doctor or in some cases pharmacist and follow his/her advice.
Unfortunately I'm not clear on your questions. I'm going to assume that you meant to say that your syringe is 1cc or 1ml capacity and the capacity lines marks are in U-100 units and go from 10 units to 100 units. I'm now also going to assume that you were instructed to use .2ml which should translate to 20 units. If that's the case you should fill to the 20 mark on your 1cc/ml U-100 insulin syringe for the .2ml. If you're wanting to try .05ml you would use half of the 10 unit mark or count 5 of the unnumbered 1 unit marks (again, I'm assuming your syringe has those).
Even if your 1ml U-100 syringe has marks labeled from 10 to 100 it should still have smaller marks indicating single units even if they aren't actually numbered. Just count 5 of the smaller marks. However, you really should be using a smaller capacity syringe. The ones I use are just 1/3 or .3 of a cc/ml and are numbered from 5 to 30 units with smaller marks indicating 1 and 1/2 units, although these aren't numbered.
I'm also assuming you're injecting manually and in that case for doses smaller than25 units I suggest .3cc/ml syringes. If you're using an auto injector you'll have to use the .5cc/ml syringes as I think the .3cc/ml syringes are too small for the injector device.
As your question wasn't clear I made a lot of assumptions, which isn't at all good. Please clarify with your doctor or pharmacist.
