After much consideration, I decided to get an implant. It's just enough. I've been living with premature ejaculation and erectile dysfunction and venous leaks for almost 9 years. you story sounds pretty similar to mine. my dick also only worked when on my side!

partners wont notice the reservoir inside of you. its too deep to be palpable. only thing they would notice is you pumping up your penis.

mine is a titan, so in the earlier stages it does make a noise, its easily masked with playing music.

i never had a partner notice unless i tell them. you would need to take some precautions and come up with your own method but its definitely doable.

Yes, maybe some blood vessels are pinched in a way to reduce outflow in this position. Common for certain positions to cause better erection.
