New Coloplast Titan with Dr Taj

Greetings,

I am a 60 year old diagnosed with venous leak. I had 24cm Coloplast Titan implanted by Dr. Tajkarimi in Northern Virginia on July 17, 2025. He did a fantastic job and I have had somewhat unusual recovery.

Preop, I was about 16.5 cm in length and 15 cm in girth. Starting around 55, I was having progressive difficulty getting and maintaining an erection. I started with 25mg Viagra and initially that was effective. But as time passed, it became hit or miss even with 100mg dose. I made a consultation with Dr. Taj and he recommended on getting an IPP with Coloplast Titan. Little over a month later, I joined the bionic tribe.

As soon as I awoke from anesthesia, I noticed throbbing sensation at the surgical site, but no real pain. I had no bruising or scrotal swelling what so ever. First day, I had ~70cc bloody discharge from my drain. I had no problem urinating, although it felt like urinating with moderate erection. I did not take any pain meds.

I saw Dr. Taj on POD 1. He took the penile dressing down and went through a cycle. I had no penile swelling. On POD 1, I had ~50cc bloody discharge. I did not need to take any pain meds after the procedure.

POD 2: I started mild cycling keeping it at 70% and dropping down to 50% to urinate. ~25cc bloody discharge in AM and ~25cc serosanguinous discharge in PM. I noticed that tip of implant in glans was quite tender when poked. No need for pain meds.

POD 3: I had ~40cc serosanguinous drainage. I started moderate cycling going up to ~80%. Penis was still covered in dressing, but glans appeared normal with no swelling.

POD 4: I had the drainage catheter removed by NP. She had me go through a full cycle. Once I got home, I went through a complete cycle. When fully or partially inflated, implant felt crinkly and penis looked ugly. It was freely deformable like a putty. It also felt like there were multiple air bubbles in the implant. When inflated to max, there were hard knots in mid penile shaft on both top and bottom as if there was a twist in implant. I contacted Dr Taj and he said, all Coloplast Titan were like that in the beginning and it would get better over time and cycling. I was worried.

POD 5: Went through full cycle and knots were noticeably improved. I did a measurement and I was 14.5cm in length (down 2 cm) and 14 cm in girth (down 1 cm). My theory is that when implant is not fully distended, there is going to be buckling in the implant that is felt as knots. I was probably better inflated today leading to less knots. As I had no pain at ~80% inflation, I tried oral and PIV sex just to see if I could. I didn't finish, but I felt pleasure and had only minor discomfort when penis was flexed. I tried cowgirl, reverse cowgirl, doggy, and missionary and didn't have any discomfort. I'm not going to try anymore sex until staples are out in 3 weeks.

Today is POD 6. I went through a full cycle and kept it at max inflation for about 1 hour. I think even at max, implant is probably only about 80% inflated. My measurement today was 14.5 cm in length (down 2 cm) and 14.5 cm in girth (down 0.5 cm). Knot is even less noticeable compared to yesterday. Only medicine I have taken since the operation have been levofloxacin (antibiotic) and gabapentin. I am hoping with aggressive cycling, I can get my length and girth back. Only other concern that I have is hard implant tip in the glans. Unless my glans is engorged, I think it may be painful to both me and my partner during sex.

Overall, I am very satisfied with my implant operation and post operative course. I am hoping over time, implant will get softer and less crinkly. I am looking forward to a day when I am banging 3 times a day without worry.

It's an exceptionnal recovery, congrats!

But as you can read on several posts, be patient everything will settle down itself.
46 yo, ED since 3 years, tried pills. Venous leakage. Implanted 30/04/25 Pr. Andrianne CHU Liège coloplast titan OTR 20 cm +2 cm rte.
Initial measures ( ved 17.5 cm length, +-16 cm length +- 16 cm girth, 4weeks post-op 16cm length)

Congrats for your successful implant. Good to hear that your recovery is going well. As Nico stated, be patient, you will be using your implant for many years.

You are very early in the process!!! In time it will soften up a LOT! Mine took 6-8 months to get what I would call soft. By a year it was very normal feeling with a few pumps in it to make the cylinders just full with no pressure. Been just over 3 years now and it’s as normal as I can ever remember pre surgery. Took many months to get used to being a shower but after a while I can’t remember what it was like being shriveled up when not excited. Love my bionic member and have absolutely 0 regrets!!
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast with Classic pump 20cm and 1cm RTE. Going strong

There's a post a few months back titled: "Trading in new Titan for AMS after 90 days."
The guy just couldn't handle the "crinkly" feeling of the Titan so he had it ripped out.
In the post, some guys stated that the Titan got softer, while others stated it was still uncomfortable after a year. Might be a worthwhile read for you.

POD 7

Length: 14.5 cm (-2.0 cm)
Girth: 14 cm (-1.0 cm)

I took a long walk and outing today for the first time since getting the implant. It was interesting walking with a bulge. I was probably 40% inflated and still had quite a bulge. It wasn't uncomfortable, but definitely like having a semi-hardon from my youth. Still no pain and surgical site looked good. I changed the dressing over the drainage hole and there was no discharge on the dressing. I am now done with post op antibiotics and showing no signs of infection (knock on wood). I am cycling to max pain during morning shower and then ~80% whenever I am home, only taking it down to ~50% to urinate.

One other thing with the implant. When partially inflated, I think there are air bubbles in the implant. Will these eventually make it up to the reservoir with repeat cycling since that's the highest point. I think I read somewhere that reservoir and possibly implant are air permeable and air will eventually exit the implant system. Is that possible or true?

Wow, your progress is amazing. What also amazes me is the incredible variations that urologists have when giving postoperative instructions.

I am day 4 post op and all I am doing is locating the pump and gently jiggling and pulling it down. Aside from that I have to gently massage dick and ball sack and keep it pointed upward. I do not start the cycling process until week 5

I have no idea or opinion as to what approach is best practice. I just find it interesting the significant variation in the approaches taken.

I cannot think (not that I’ve given it much thought) any other medical procedure that has such almost polarised approaches to postoperative instructions.

Anyhow, best of luck to you.

Several ways to look at that subject. In the surgery videos that I've seen. The dr is very careful when he measures the saline. Any air in it can't be very much. Also I've read as you have that all or some of the system is porous to let air migrate out.
It's highly unlikely that you can detect any air while you are pumping. Air starts migrating out as soon as you're stitched closed.
