23M, 6 Years of ED/Venous Leak — Day 1 After Getting a Coloplast Titan 20cm IPP

TLDR:23M, 6 years of ED/possible venous leak, poor response to PDE5 inhibitors. Got a Coloplast Titan IPP today after deciding I didn’t want ED to control my social and sex life anymore. Also had ~20° downward curvature that made missionary sex difficult. Surgery went mostly fine, but I woke up near the end with excruciating perineal pain, possibly from the catheter. Still in significant pain 12 hours later. Catheter comes out tomorrow. I’ll keep updating.

Hi everyone,

I’m a 23-year-old guy who has been dealing with ED, or what I believe to be venous leak, for almost six years.

Today, I finally underwent IPP surgery and received a Coloplast Titan.

I’ve had an extremely poor response to PDE5 inhibitors. I also previously underwent a Doppler ultrasound after a Caverject injection, the erection is not optimal as well. The results showed that my dorsal artery appeared to open prematurely. Based on my symptoms and the test results, I believed that venous leak was the main issue.

The final factor that pushed me to go ahead with the implant was my girlfriend. She knows about my situation, has been extremely understanding, and genuinely does not judge me for it.

The clinic I chose is one that has been discussed a bit in Asia and on FrankTalk. At least from my perspective, I consider it one of the top clinics in the region.

I discussed my situation in detail with the surgeon. I also had around a 20-degree downward curvature, which I really wanted to improve. It made sex, especially missionary position, quite difficult for me.

The surgery itself was mostly fine, but near the end I woke up during the procedure. I suspect this may have been because of the catheter. I suddenly experienced extremely severe pain deep in my perineal area, together with a very strong urge to pee and shit , even though I had nothing to pass.

I did not feel nauseous after the anesthesia, and I felt mostly normal again about 30 minutes later. However, the severe perineal pain was honestly unbearable. Even now, around 12 hours after surgery, I still have significant pain in the same area.

At first, I was worried that the implant might have been placed too deeply or that something had gone wrong. However, the clinic told me that the pain was most likely caused by the urinary catheter. They scheduled to have the catheter removed tomorrow, and I’m hoping the pain will improve significantly after that.

I’ve also heard that catheter removal can be painful, so I’m not exactly looking forward to it. I’m hoping they can at least give me some local anesthetic or something to make it more comfortable.

Some people may wonder why someone as young as 23 would choose to get an implant.

My personal view is that I wanted to get my social and sex life back.

For years, whenever women showed interest in me, I would find excuses to avoid the situation because I was afraid of what might happen sexually. I got tired of constantly having to hide from intimacy and make excuses.

I know an implant is a major decision, especially at my age. But I also believe that problems should be solved regardless of age. I would rather potentially need more than one implant over the course of my life than spend my early twenties avoiding sex, relationships, and experiences because of ED.

I know this decision is not for everyone, and I’m not trying to tell other young men with ED that they should get an implant. This was simply the decision that made the most sense for me after years of dealing with this problem.

Hang in there! Day one and about any random pain is normal.as far as catheter removal, it never hurt for me. Very uncomfortable overnight but removal was more a relief. Nurse deflated the balloon inside and said take a breath and breath out slow. Just slides right out. Kinda a weird feeling but not painful by any means.
60yo.ED started mid 40s pills failed after 10 years. Injections works w diminishing results.
Implanted 5-22 Titan 20cm with 1cm RTE.
Revision 3-26 Titan 24cm with 1cm trimmed off.
Infection, salvage 5-26 Titan 24cm -1cm trimmed.

Welcome to the bionic brotherhood keshiwo. Your decision was clearly well considered and does makes sense for your situation, despite your youth.
You obviously have also considered that it's likely you'll need several revisions during your lifetime, but technology is evolving and hopefully there may be more permanent solutions available by the time you get to that point. In the meantime recover well and look forward to using your implant in the coming weeks and months. I'm sure your girlfriend will love it :D.

With the catheter, in my experience removal is very quick but you may have some soreness afterwards that should go away fairly quickly.
And don't avoid using ice and pain meds to stay ahead of any discomfort as needed while you are healing from the procedure.
72, married, Sydney Oz. PC/nerve sparing RRP Mar 2022 caused 100% total ED. Tried pills, Trimix inj, focal shockwave, VED. All failed. Implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal. Got back to 6.5" BPEL @ 9m. Loving it.

24? Man, I forgot that detail. I wish I had known about this when I was 24. I would have done it immediately.
Born 1990. ED since age 20 after a bicycle accident. Coloplast Genesis malleable implanted December 2024 by the great Dr. Laurence Levine in Chicago.

Post-op Day 2:

After the catheter was removed from my body, the perineum pain basically went away immediately. But the reservoir started hurting whenever I needed to use my core to move, like getting up from bed. My glans was also pretty swollen because of my bad dressing. I could feel my dick getting more swollen if I stood for 15 minutes.

Day 3:

The reservoir pain kept getting better and better, but a new pain showed up: skin irritation/burning/needling sensations around the bottom of my shaft and scrotum. At first, I thought it might be because of the "dog ear" thing, but it turned out my surgeon said it was just skin/nerve irritation.

Days 4–6:

Basically the same as Day 3. The irritation is getting better, and the glans swelling is starting to improve as well. But now the pump is tugging on my ball/scrotum. For example, when I finish taking a shit and stand up, it feels like someone is kicking my ball. It's different from the irritation pain, but it's still really hard to tolerate because it makes my ball ache.

Oh gosh, I really wanna jerk off now.

Yeah indeed, I barely felt any pain from the catheter removal.

Yeah, I totally understand that I might need to go through a couple of revisions in my lifetime. But still, if you told me I'd have to give up enjoying sex with my partner and avoid having a normal sex life, I'd still go for the surgery without a doubt.

And I also have PE myself, so it basically solved both of my problems in one go, haha.

Congrats on taking the step at your age. Yup, the first few days/weeks can be very uncomfortable.
Makes you ask yourself, "what the hell did I let them do to me?"
