Anyone here with UHC, and if so, how much did they cover?

My husband and I are both hoping to get the Titan from Dr. Hakky in Atlanta this Fall. We're on my United Health Care insurance, which has a co-pay (% of total bill) and an out-of-pocket ($3000-$4000). We're waiting to hear back from UHC before we commit so we aren't stuck with a huge bill we aren't really ready to pay. (We both really want this procedure, but my job situation is tricky these days so we're being careful, and neither of us have MediCare yet.) I know every policy is different, but for those who also have UHC, what was your ballpark range for coverage?
