Will try and keep this short. Over last 5-6 years, I have seen a slow deterioration of my erections. Very sexual guy, not unusual some weeks to have sex everyday. This deterioration corresponded with me going on HRT, and so I initially wrote it off as fluctuation in estrogen and T. Added daily cialis, which helped for a while. Then two years ago, started Trimix very reluctantly. As a matter of fact, just used it 7 times in 2 years. The first time, followed the prescription, went low and didn't get much out of it. Second time, did max dosage on instructions, and ended up in the emergency room. Out of the next 5 times, 1 success and another priapism this past May, in hospital again. Never found a safe dose, .01 variation could mean a limp dick or a trip to the ER.
This time, did not bounce back and saw a real deterioration. Getting my doppler next week, to make sure. I read on here the number of guys that waited and lost a lot of function and size, and decided not to let that happen. Getting two opinions from two Urologists but on Kramer's schedule.
Would like some advice on:
Prep for surgery - pumping?
Choice of implant, any real difference between AMS and Coloplast?
Pump placement, do I have any real choice?
Cialis use after. I do get use out of it in the gym, and advised that it helps keep glands full. Experience with this?
Any thoughts would be appreciated. In the DC area.

All of these aspects have been/are discussed on here...time to do some reading
But in short there are as many different experiences as there are men discussing them.
I did no prep.... I didn't watch any surgery videos and the only stretching I did was jam a Caverject in the night before and pound the bejabberwocky out of my beautiful GF.
Choice of device? AMS seems to be more natural flaccid but according to some who have experienced both AMS and Coloplast the Coloplast is much harder at full throttle...i can vouch for this because I am using it daily on the heavenly Miss H.....again there is a lot written about the AMS vs Coloplast on this forum.
Pump placement:
Mine sits centred and at the bottom of my ball bag.... Perfect....this is probably dependent on the experience and skill of the installer as is choice of length of implants.
I am 150mm flaccid and 170mm inflated with a girth of 142mm and slowly increasing through twice daily inflation to the point of excrutiation!
If you choose a Titan, the research suggests 2 benchmarks for size-one at 12 months and a final at 24 months... Provided you inflate to pain level twice daily.
At week 8 i bought some Viagra because I read on here about glans engorgement.... I've taken 1 only....it reminded me why I got my Titan! I haven't taken one since.... Miss H and I have an agreement... She sucks my cock for at least 15 mins to get my glans fully tumescent and in turn I pound her into orgasm heaven for as long as she desires.... Works for me!!

I just got my implant. I really don't think there is to much of a debate on pumping. I did it, I don't think it hurts. Some doctors inject you to get your size. I pumped the nite before marked the pump with a pin and handed it to my surgeon. I think it is your choice. I think what ever your doctor tells you he is comfortable with implanting. Both work both well built.I was never asked about pump placement. Cialis? Again your call. My 2 cent

Sounds a lot like my story. Yeah, I had a trip to the ER when my erection was still rock hard after 5 hours, and it was at 2am. Talk about embarrassing. Did have a sexy young black female doctor though. I even tried something called PT 51. This is injected. Gave me random erections all day long.
I think you are right, going by my experience. It doesn't get better, so way prolong it.
As far as pumping, my doctor told me not to pump. As far as choice between AMS and Coloplast, I have heard that there isnt much difference. Originally I wanted to go with a Titan, but my doctor only works with AMS. So that is what I got. I can't answer about pump placement. I had hernia surgery, so I had to leave that up to my doctor. I was wondering about the cialis use myself.
You will get a lot of good feedback here. Welcome.

Read and read some more. FT has a wealth if info from a good bunch of caring brothers that have been there. Educate yourself & you will have the knowledge to ask the important questions when you meet with doctors. As far as which device thats something you need to discuss with doctor based on your situation & anatomy. I used a pump 4-5 times a week since my RRP. Increased pumping the last 6 weeks prior to my implant. Definately think it helped me to stay flexible. if you do research you will feel more confident about your decisions. Good luck !

Doctor choice is the most important. Do your homework and go with who you are most comfortable with. There's a lot of good ones mentioned on this site. Once you choose, following his advice would be well advised.
Good Luck

Guys, thanks for posting. I think in many ways I am lucky. Found this forum, which helped a lot. My issues really came to a head this past month, so I am getting on it as fast as possible, so I think I haven't had to go through what some of you had to. Though I admit, the very first time I doubted whether I could get erect, man that was the worst.
Almost every posting I read ends with the statement that the guy wish he had done it sooner. I am lucky that Kramer is close by, and I am also getting a second opinion, just to be sure.

Re: Scheduled for August 23 - Any Advice would be appreciated
Dcmusc,
This site has been a terrific help to me. I had increasing ED for years, and the pills worked well for quite a while. Then slowly the erections wouldn't last, and then they would sometimes not appear. My wife was very "understanding" and supportive -- but of course women can't understand it all. The cycle of depression-ED-depression-... Is brutal.
I was diagnosed with prostate cancer 1/17 and had a robotic RP 3/17. Lost over an inch off my already small dick, and zero erections. Tried pumping to rehabilitate and injections for rehab and intercourse. Was barely successful twice, but highly un-satisfying foe either of us. The pain with the trimix was terrible, the bimix not so bad.
So I finally bit the bullet and scheduled a consultation and surgery with Dr. Kramer on August 1 and 2. He said to stop the pumping and only inject for lovemaking, not re-hab. I know a lot of guys (and their doc's) recommend pumping prior to surgery, but I'm going with another piece of advice that I've read here many times -- Trust your doctor and follow his or her directions. Think of how much trust we are placing in these surgeons for the implantation. I'm not going to try to second guess him.
Going on two week vacation starting next week. Vacation typically means sex every day. Wondering what the little vials and syringes will deliver.... The big day will be two weeks after I return. I am excited and nervous as hell.
All of the sharing that goes on here helped me make the decision that was inevitable sooner rather than later. The recurring theme of "only regret is that I didn't do it years earlier" got me to move. Have to admit, the recurring theme that the first week or more may be worse than no fun at all scares me. Sounds like the recovery will be more intense than that from the prostatectomy. But the results should be well worth it.
Still hard to believe I am actually going through with this. And thanks to all who helped me get this far.
Glad you're scheduled with Dr. Kramer. We can compare notes here before, during and after. Wish you the best.
