Concerned about upcoming op

Postby Matthew » Sat Aug 18, 2012 11:56 am

I meet Dr Milam Monday for consultation. I am tentatively scheduled for Sept 12 implant. I have read about positive and negative outcomes from these great dr's. It seems some come out very pleased and close to original size and others get short changed quite a bit in size. My question is: are we totally at the mercy of the doctor for what we get? I mean I am getting 6.5" and good girth with VED. I do not want to be at 5" or so with less girth. Do I have any say in what size unit is used? I understand I cannot oversize myself. What I see is a pattern of many doctors being too conservative (for there own benefit and loss of patient benefit). I am married to an attractive woman who has good libido. She has always complimented my size. I do not want to ruin that becuase a doc is looking out for himself more than me. I would be happy with 6" and good girth. Can I tell him I want at least a 6" set up with girth at least 5"1/4"? I just am struggling with having little or no say. Is that the case? Can I sign off liability to a certain size after he evaluates me? thanks

Be very vocal with your surgeon about your questions, concerns, and expectations. I would say that if you were having a hangnail fixed. When it comes to your most important body part, it goes double.

Matthew,
The "Rule-of-thumb" is to grab your penis and pull it straight out as far as possible ... that's what you'll get with Implant. You can not go by what size you get from VED.

No you can not dicate to the Doc what size "you" want ... the Doc will measure with a special tool from within during surgery ... there are many videos at Youtube that show the Implant operation.

Ask yourself ... would you rather have a working member ... or just one that hangs there looking pretty? Better yet ask the wife!

Have you engaged in the penile rehab exercises two months prior to surgery as described in this research (Urology Times) article? It works.

It would appear that the measurement of pulling the penis out like mentioned is a correct way to measure. The issue is if the doc is saying ok I pulled 20cms. I will err to 18 or whatever. This must be happening in some cases. That liability for the patient is not something the dr is liable for. Would want to see a video of the measurement during the op and then see the model size #. There has to be accountability. Some are losing over an inch. That is unacceptable.

you are correct, you will have no input into the sizing, the doctor will make an incision in you corpus cavernosum and dilate it, measure the distance to the tip and the distance to the end of the base and the sum of these two will be the total lenght, the part you see and the internal part. If he adds more lenght than you have space you will be a canidate for failure caused by errosion of the chamber, usually at the tip, you will be up the creek if that happens. If it is too short then you get floppy head effect and that makes your unit much less effective. Point being, it must be sized to fit you, in other words they measure then choose the parts to install, there should not be, "hey the guy wants this" or that. You need to accept that and more importantly you need the have a doctor that you trust. In fact it is or should be pretty much pointless even to discuss the lenght issue with the doctor. He will or should only do what the facts indicate. If you are not comfortable with that then you might want to reconsider this route. It will almost surely be shorter than your ved lenght, it should be close to your orginal natural size. On the girth issue, you can have input into this issue. The girth you mentioned could be produced by the AMS 700 LGX and with the same unit my lenght after I first began inflating it, has finally streched out to the very near my orginal natural lenght. This process took almost a year for inflating to the max for about an hour every day. THat's right, there will be work to do before all is finished.
All said, if you are able to use the ved, I would keep doing it till the implant issue was a "no brainer" type decision, then lenght might not be as big an issue for you, at least it wasn't for me, hey, these things really work!

"the doctor will make an incision in you corpus spongiosum and dilate it, measure the distance to the tip and the distance to the end of the base and the sum of these two will be the total lenght"

When i think of dilating, I think of girth expansion. Do they stretch it after they cut into it to get the lenght measurement?

no, the "dilator" they use is a blunt ended metal rod and there are graduated measurement marks, they force the dilator to the end of the cavernosum, that is the streching used for measurement. THe "grab an pull" they do in the office for you is just to give you a rought idea of lenght. If you need the implant, you shouldn't fret about lenght, a good doc will make it as long as it can be, he is interested in a good outcome also, too short causes problems as well. About the dilation, they basically use the rod to hollow out the internal structure so the implant can be installed. Do a google seach for the video done by dr. Ford in Shreveport LA, it shows the actual operation. Good luck

Matthew, I had some of the same questions and concerns. As a retired psychologist, I also have dealt with size issues from that side as well. You will be best served by trusting the doctor. I inteviewed probably a dozen or more before deciding to have it done. I was comfortable with all of them. I do not think that "under-sizing" is very common or exists at all. As previously stated, too short is a formula for failure and docs do not want that. In truth, you will be measured for what is medically and correctly appropriate ant the time of the surgery by the surgeon using instruments designed exactly for that purpose. Also, there is some expectation that you will regain some length along the pathway to recovery. No guarantee of that but several on this forum report so. In the final analysis, you need to be open to the outcome and willing to trust your medical team. I lost about one inch, but I also have gained about 2000% in self-esteem and general happiness to know that my days of wondering about performance are in my past.

Good luck on the procedure and keep us informed about how wonderful it turns out to be.

"I do not think that "under-sizing" is very common or exists at all."

" I lost about an inch"

These two statements are condradictory to me. I do appreciate the honest feedback.
