I suspect that what you’re talking about happened to me. Though not with Cialis and Pentox, but rather with trazodone. It’s an anti-depressant that is also prescribed for insomnia, which I have.
I took 50 mg at night for sleeping and it worked well. My sleep schedule was back to normal. My doctor said a rare side effect is priapism and at the time I joked “4 hour erection, doesn’t sound like a problem to me ha ha” but then I developed painful erections then Peyronies, so here we are.
The first uro I went to said trazodone is no good, most uros refer to it as “trazaBone” because of what it can do to penises.
So maybe I’ll phase out the Cialis for a little bit and give my NTEs a break
Thanks for the info

Yes, I couldn't agree more.
I am and always have been a firm believer in combination therapy (PDE-5 inhibitor + oral phentolamine in my case, I never tried Pentox) but, and I can't stress this enough, there is an increased risk of side effects and priapism is definitely on the list.
I don't like daily treatments for ED, even in small doses. I used Cialis for more than 10 years with excellent results but only as needed. Daily combination therapy sounds even more controversial to me.
Did you doctor say why he chose Pentox? Do you have any penile curvature and if so, have you seen any improvement?
Age 40. Psychogenic ED for over 20 years. Current regimen: Udenafil 200 mg, oral phentolamine mesylate 40 mg, Seredyn.

Although these self-terminate and are not deemed "major priapism" due to the shorter duration, they still share the ischaemic, low-flow phenotype with the major attacks. Furthermore, it is thought (although I'm not sure completely proven) that the recurrent nature of these episodes can lead to multiple small ischaemic insults that over time can lead to cumulative damage to the corporal tissue +/- possible smooth muscle injury/fibrosis. This thinking is based on the fact that they have seen patients with stuttering priapism develop ED over time even without having major priapic episodes lasting over 4-6 hours.
Very interesting issue. Not familiar with stuttering priapism. Let’s hope this has not occurred in your case.
Many years ago when I first started T therapy and I think my dosage may have been excessive, I noticed that I would wake with very intense erections in the night and I would have to wait for a while before I could urinate. The erection would take some time to subside. Night time erections are very T dependent and are speculated to be stimulated by T and the fact that the inhibitory influence temporally shuts down during REM, effectively removing the brakes! As men age T levels decline and the inhibitory influence increases, not a good combination for continued good erectile function!
After reading your post I stumbled across this article:

It was really interesting that paradoxically, Tadalafil was used to help correct the stuttering priapism issue in the above patient, resetting the balance of the NO and inhibitory pathways, by up-regulating PDE5.
Wait to hear what your tests show.
Age 57, ED issues for 15-20 years. Testosterone replacement with Enanthate and Ovidrel. Currently using generic Tadalafil 2.5mgs and Resveratrol daily.
