To lessen the pain use as little as necessary and no more. The addition of a novelty cock ring can help a bit to allow you to lose less but you would have to experiment with different rings and expect rejects. If it's too tight your wearing of it will be time limited to 20min or so. Some guys have reported success with combining Viagra or one of the other oral drugs with an injection to enable a smaller alprostadil dose. Consult your doctor first before trying that. Always use caution and start low. An insufficient erection beats an ER trip. I agree that using the least number of drugs possible is usually best. It is still my understanding that alprostadil, in the form of Edex or Caverject, are the only FDA approved erection injection drugs. It's also my understanding that alprostadil has the least potential risk of scaring and fibrosis. However, there is some risk with all of the injection drugs.

Yes, in many cases the alprostadil pain does moderate or disappear completely with continued use. But not in all cases. I don't know how long in general it takes. I also initially had some alprostadil ache, but probably not as bad as some. The ache didn't go away all at once and took quite a few months. Part of it was learning my correct dose and injection techniques. I didn't know about Frank Talk and my Urologist was of very little help so my learning curve was probably longer than necessary.

Your question has little meaning without knowing the strength of your mix. What matters is how many mcg of alprostadil you're injecting. That involves a little math based on the strength of your mix and the volume of mix you're injecting.
I started with Edex 10s, which means 10mcg of alprostadil. It didn't get the job done. I then went to Edex 20s, which after improving my injection technique, worked too well if I injected the full cartridge and they produced a notable amount of alprostadil pain and longer than desired erections. But I hated wasting the juice so injecting less than the full Edex 20 was difficult. I was banging on the go to ER door fairly often but never made that trip.
Around this time my insurance company quit covering Edex altogether. They rationed them pretty severely initially but then quit covering them altogether. That left me scrambling. I then got my GP/Primary doctor to write me a generic script based on the Edex ingredients but much stronger, equivalent to what would be an Edex 80, if they made them. I then found a compounding pharmacy, somewhat locally, that would fill the script.
After that point I was able to finish perfecting my injection techniques and perfect my dose. I also experimented with and perfected my technique with novelty/sex shop cock rings allowing a further reduction in dose. I now get no alprostadil pain. I also experimented with combining with Viagra. I found that the combination also allows less injection juice but I eventually decided the benefit it provided wasn't worth the heartburn.
I'm currently using 14 units of an 80mcg per ml mix aided by a silicone cock ring. Please note that these rings aren't remotely as tight as what medical grade rings would need to be if I was trying to maintain an erection produced with a VED.
Yeah I tried depending on a VED with ring before going to injections. Of course the first step was trying pills, as it should be. They gave me NO erection regardless of stimulation. The pills didn't work at all by themselves but they do help with pumping with a VED and with injections.
