Update: First Cycling/Activation of the Implant, exactly 3 weeks since day of surgery.
(Dick pics warning below)
- Still had some swelling in my balls as I had minor clotting/hematoma in there, but decided to say "fuck it" and try to activate. The pump was rock hard and wouldn't budge; found a thread on here that said getting in the bath/shower helps relax things.
- After about 5 minutes of running warm water on my junk, I squeezed as hard as I could on the pump and it finally gave. I pumped about 10 (give or take a few) in total I'd guess, and voila -- my first bionic boner. There was a fair amount of pain/discomfort (went away quickly after) so I couldn't really get my glans engorged, so with a flaccid glans I measured in at about 6.3" -- vs original 7.1" pre-op length. Keep in mind that's with no glans engorgement.
- Well..,this next part of the update isn't easy for me to write. My concerns around implant positioning, as I suspected, remain. The implant tips did not move very much farther up my shaft, if at all. They are still not in my glans -- as a reminder the requirement for the right rigidity and support of the glans is for the cylinders to *end* 1/2 to 2/3 into the glans. Currently my cylinder tips are 0% into the glans, even when inflated. I know I have to keep cycling, but I continue to have the strong belief that I was undersized and it feels like I need a miracle at this point.
Still holding out hope that I can be like Robocock, who had this problem and corrected it through cycling somehow. That being said, my understanding is that the implant tips *should* be in the glans at this point, and I am mentally prepared (if not financially prepared, after paying a full year of savings for this one out of pocket) for looking into getting a revision, based on what I've read about this issue.
Also, to note: to the guys in this thread who pointed out that their implant is not in the glans when flaccid but is when erect, I did some research and (with the exception of Robocock) it looks like you have the AMS, not the Titan -- the Titan does *not* expand in length, whereas the AMS does, so our devices behave differently in that respect. This contributes to my concern. I appreciate the comments nonetheless.
Anyway, pics below. Still some dry skin on my junk from the scabbing from the surgery, didn't want to peel it 'cause it's still tender in some places. And that stuff on my balls/incision is a form of heavy duty, gluey stitching that the doc used since I was travelling out of country and wouldn't have a doc nearby for regular suture removal.

Did you have the famous "snake eyes" after surgery?
If so, the doc probably had to pull the tips into the glans,
When you inflate, see if you are able to pull your penis back to make the tips go into the glans.
Maybe you do that when flaccid?

The surgery was performed by one of the top US surgeons recommended by this site. I don't understand how he could have missed something this obvious. I'm almost certain now I'll need a revision, in which case I'm fucked because I just spent my life savings on this implant.

Please, could you say the name of your surgeon? there are many people who have save money with a very big effort who are thinking in travel to EEUU to get an implant and the name of your surgeon can help us to do the best choice and avoid problems like that...

Let's say worse comes to worst and after many weeks of cycling, you find that the cylinder tips still don't extend as far as they ideally should. When this happens, the two biggest concerns are, one, obviously, loss of length, and two, floppy glans.
So let's deal with those one at a time. First, the length. It's hard to tell from the pictures, but your dick length looks at least average, probably above. Even if it ends up being slightly shorter than your peak pre-surgery length — and remember, you have many months of growth ahead, regardless of where your cylinder tips end — it's not like your surgeon turned you into one of those "short, short men" that women sing about not wanting. You have more than enough to satisfy a woman and make her cum.
As for the floppy glans issue, I can tell from your pictures that you're still getting good natural glans engorgement, which is likely a function of your age. The glans has a different blood supply from the shaft, and if your surgeon did his job and kept that supply intact, you don't necessarily need the cylinders in your glans to stabilize it during an erection — your natural engorgement will do that for you.
All of this is to say, the last thing I would be doing if I were you is freaking out about a possible revision right now. Your implant works, your dick will now get hard whenever you want and for as long as you want, and you have the size to satisfy a woman (even if it's less than you envisioned). Those are the important things. Worry about a revision hopefully MANY years down the road when your implant starts giving you issues.

Look at post from Tmansdorfer, floppy glans. Look at his pictures. That is real floppy glans. If you cant bend your glans over the top of your implant like he does, you don't have a problem. My implant moved considerably into my glans in the first year, and I have no blood flow at all in my glans. You will be ok. Give it time.

The simple fact is nothing can be done until you are healed up and several months pass from your initial surgery.
I had both titan and ams 700. I have noticed no real difference length or size between them. Not pumped my tips are barely in glans on both. Pumped they both go well up there. As the cylinders soften both get more length.
I was able though not pumped to push my glans down onto the tips and feel them at the tip of my glans. Pumped up they both moved where they should be.
Cycle, cycle and cycle. I stayed pumped up my first year an hour a day at least and sometimes 2x a day. Once in morning once at night.
I had both titan and ams 700. I have noticed no real difference length or size between them. Not pumped my tips are barely in glans on both. Pumped they both go well up there. As the cylinders soften both get more length.

18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

Robo - I truly do appreciate the positivity and optimism, I really do.
So what I'm writing isn't meant to put down your points or be combative, it's just the reality of my situation: my glans engorgement is still not great. A large part of the reason I got the implant was because of that problem actually; it doesn't maintain without constant stimulation, plus I have terrible PE, so I was really relying on having the rigidity from the cylinders right up into my glans. Without that, I may as well have not got the implant.
Even people *with* good glans engorgement but who don't have proper cylinder placement (like ViaSwiss, who has a super similar story to me) is getting a revision done. So this isn't something I'd just live with. If the cylinders don't move where they should, it's going to have to be a revision for me.

Hey man thanks for the comment -- I think the main difference here is you were still able to push the glans down and feel them at the tip. I can't even remotely do that; the cylinders feel firmly stuck behind my glans.
I know I'm only 3 weeks out from my surgery but I still have more questions than answers about this. My 1 month post-op with the doc is coming up, but I'm very pessimistic about my outcome for a few reasons:
- ViaSwiss is basically in an identical situation, just a few months ahead (his pics and cylinder placement are almost exactly the same as mine at this stage). He has mentioned that almost all of the best surgeons he reached out to regarding that issue said that the cylinders should be in the glans by this stage (yes, even by 3 weeks in).
- Other men with Titans (with a few exceptions like Robo) were able to feel the cylinder tips the necessary 1/2 to 2/3 into the glans from the very beginning. I'm nowhere near that, because the cylinder tips are 0% into the glans.
If anyone else with a Titan (not AMS) recovered from this situation I'd love to hear more about it. But it's really not looking good. I can post more pics, it's full on floppy glans.

Hey Eustinas, so sorry to hear about the issues you're having. I genuinely hope you are able to get your issue sorted.
I am in a very similar situation that you found yourself in leading up to your surgery--i'm a young man living in ontario hoping to get an implant but I would like to go to the states to get it done by a high volume surgeon as opposed to the surgeons that are limited by gov't funding in ON. That being said, would you be willing to share the name of the doctor you originally went to? Not necessarily to name & shame but just as additional information shared with this board for others who come through and are doing research on when, how and with who. I'm currently trying to decide who I want to get my surgery done by and this info could have an impact on my decision.
If you would be more comfortable PMing it to me that would be great as well.
Again, I'm hopeful that you'll be able to get your issue sorted. Hoping for the best for you!
