I am a healthy 69 year old who had a radical prostatectomy in 2005. I had 3 goals in mind at the time: life, liberty and the pursuit of happiness. On the life score, I am still here and my PSA readings have been undetectable since the surgery. On the liberty score, I am free of incontinence but have some chronic urinary issues with reduced flow and emptying my bladder. Regarding pursuit of happiness, after five years of frustration with pills, pumps, injections and abstinence, our sex life was restored following a penile implant in 2010. I have no regrets, but can’t say that it has been an unqualified success.
My implant is a Titan Coloplast OTR. I didn’t taken measurements before or after but guess the length and girth of my erection is now about half of my natural size. The OTR is supposed to mean one touch release but that has not been the case. It takes a hard squeeze of the release button for at least five seconds to initiate the deflation. After that, it occasionally requires an additional squeeze of the penis to fully deflate later. Pumping it up takes 15-20 similarly hard squeezes so I worry about having enough finger strength and dexterity in years to come. As others have noted, the head does not inflate and due to the prostatectomy, there is no semen but I do have orgasms. We have found a few positions for intercourse that accommodate the smaller erection and my wife doesn’t complain – but it sure ain’t what it used to be.
In flaccid state, my penis is larger and firmer than its natural state. The cylinders are noticeably kinked in places and the end of one cylinder protrudes to the side, just below the head. This results in the head being permanently skewed to one side and the immediate area was sensitive and painful for more than a year after the surgery. I have a fair-sized scrotum but the addition of a third object within has resulted in chronic discomfort and sensitivity. Frequent adjustments to my clothing are necessary.
In summary, the implant is certainly better than nothing, which is what we had for five years. My recommendation to others would be to consider their circumstances and try every less-invasive alternative first.

Re: Titan OTR - Better than nothing
I'm glad ur having some success, like u said, its better than nuttin! I do believe that what you describe is not the normal. I don't believe you should lose more than 1/2" in length and nothing in girth. I also don't believe you should have the kink you describe. I have the LGX, over 2 years now. I'm now to the point it all feels normal. I can squeeze it down until it is quite flexible, though the head still had the hard sockets which every once in a while get in a pinch and cause some discomfort. My testicles have shrunk considerably due to T therapy, so there is plenty of room for my pump, and it seldom causes any discomfort. I told my uro that I though he did a great job on me, and I meant it.
